What are Community Health Programs?
Community Health Programs refers to the Affiliate’s efforts to
reduce breast cancer mortality rates in their service area by:
 understanding the communities’ needs and assets
(Community Profile);
 funding effective programs that meet the prioritized needs
(Grantmaking);
 empowering communities with breast cancer knowledge
and resources to take action (Education and Multicultural
Health); and
 leveraging relationships and building partnerships
(Community Organizing).

High percentage of women over 40 in Texas Panhandle who have
NOT received a mammogram in the last year.

Funding Priorities
1. Increase the number of mammograms provided to women
living in the Texas Panhandle.
2. Increase women’s access to breast health services.
3. Increase the number of free or reduced-cost mammograms
provided to underserved populations.
4. Increase the number of underserved women who receive
breast health information and breast health services.
5. Increase the number of partnerships – especially with rural
health providers – to provide breast health information.

C-3: Reduce the female breast cancer death rate.
C-11: Reduce late-stage female breast cancer.
C-17: Increase the proportion of women who receive a breast
cancer screening based on the most recent guidelines.

Eligibility
• All past and current Komen-funded grants and/or awards are up
to date and in compliance with Komen requirements
•Has tax exempt status under the IRS code
• Be a non-profit organization located in or providing services to
one or more of the 26 counties in the Texas Panhandle
•Project must be specific to breast health and/or breast cancer
•NOT eligible if applicant, or any of its key employees, directors,
officers or agents is convicted of fraud or crime involving any other
financial or administrative impropriety

Allowable Expenses
• Salaries and fringe benefits for program staff
• Consultant fees
• Clinical services or patient care costs
• Meetings costs
• Supplies
• Reasonable travel costs related to the execution of the program
• Other direct program expenses
• Equipment, not to exceed $5,000
• Indirect costs, not to exceed 15% of direct costs

Educational Materials and Messages
To reduce confusion and reinforce learning, we require that
grantees provide educational messages and materials that are
consistent with those promoted by Komen:

*We do not recommend monthly breast self-exams (BSE) and
therefore will not fund education programs that teach monthly
BSE or use breast models.

Why Breast Self Awareness?
• No evidence to show that BSE decreases incidence or mortality
• Possible guilt for women who are diagnosed
• Women often uncomfortable or feel untrained to complete what
is considered a clinical exam
• 6 of the 8 signs of breast cancer are visual
• Most breast health organizations, including the American
Cancer Society, do not recommend BSE anymore

Why Breast Self Awareness?
Breast models were tools used to teach the steps of BSE, which we
no longer recommend.
The models have lumps built into them; however, the lumps are
often not a good depiction of how a breast cancer tumor
typically feels – therefore they can be misleading.
The use of models to teach BSE focused on the palpation (feeling)
step of the process and didn’t address visual inspection; the list
of warning signs includes six potential warning signs that would
be noted by visual inspection.

Common Problems to Avoid
1. Deadline was missed.
2. Not following instructions.
3. The application does not have a clear focus or is sloppy.
4. The proposal has an “internal” rather than an “external” focus or
is unsound.
5. Budget problems.
6. Too wordy and/or too much jargon.

Helpful Hints
•Read the RFA carefully — follow ALL directions.
•Be innovative, realistic, specific.
•Be clear and concise. (Sometimes less is more.)
•Write clearly, use active rather than passive voice.
