o Liveon Health Portal replaces Personal Health Manager
– No historical data will transfer to new wellness portal. Members who wish to print or save
information will need to visit the site before 12-31-2012
o Expanded Health Assessment integrated with Liveon portal
– No historical data will transfer to new tool. Members who wish to print reports or
certificates of completion will need to do so by midnight on 12-31-2012
o Life Points replaces Blue PointsSM program
– Valid Blue Points balances as of 1-1-2013 will migrate to the Life Points portal within 1Q 2013
What will not migrate?

Kids' Stamp program (ages 6-12) and Teen Points program
