PACE and Winter Community Beds 2014-15 GP Training and Education Event – 15th October 2014 1 Post-Acute Care Enablement • Led by Suffolk Community Healthcare, with support from Ipswich Hospital NHS Trust, Age UK Suffolk and Suffolk Family Carers • In reach service to Ipswich Hospital NHS Trust • Case finding patients suitable for Early Supported Discharge • Providing assessment and care plan setting and up to 5 days support at home • Led by Nurses and Therapists, the team will be based with the Community Winter Beds at Ipswich Hospital NHS Trust • Going live 1st November 2014 • Live 7 days per week, case finding 5 days per week 2 Community Beds • 24 Therapy led community beds • Based at Ipswich Hospital NHS Trust, 6th Floor of Maternity Block • Led by Ipswich Hospital NHS Trust and Suffolk Community Healthcare in partnership • Patient criteria is based on the successful Baylham Model • Average Length of Stay will be between 7 and 9 days • Will work in collaboration with PACE • Live 7 days per week • Going live 1st November 3 Other Winter Schemes • Respiratory Services An integrated community respiratory service including the proactive management of COPD. Involving GPs and community healthcare teams to identify patients who are sub-optimally managed and improve management of their condition. Led by GP Federation and Suffolk Community Healthcare. • GP Streaming A Streaming Nurse will be based in the Emergency Department and will identify patients where their conditions are deemed more appropriate to be managed by Primary Care Out Of Hours. These patients will be referred to a designated GP in OOH. The Service will be active at weekends 10:0023:00. Led by Care UK. • Town Pastors Town pastors are a watchful and caring presence on the streets of Ipswich when the night time economy is at its busiest and to provide appropriate support and assistance.Led by Town Pastors Ipswich. • Moving and Handling Training Moving and Handling Tuition for Family Carers including a falls prevention focus. During this tuition SFC identify potential 'fallers'.– Led by Suffolk Family Carers. 4
