I, the parent/guardian confirm that I have arranged for the minor named above, to be accompanied to the origin airport or departure and to be met on arrival by the person named above. That person will remain at the airport until the flight has departed and/or be available at the airport before the sceduled time of arrival of the flight.
I, the parent/guardian authorize Nextjet, should the minor not be met as stated above, to take whatever action considered necessary to ensure the minor’s safe custody including return of minor to the airport of departure.
I agree to indemnify and reimburse Nextjet for the necessary and resonable costs and expenses incurred by them in taking such action.
