▪ Enclosed is a membership application for the Distributor Division. Once
you’ve had an opportunity to review these materials we are sure that you
will realize that ADMA would benefit your organization. Simply complete the
application form and return it to Headquarters. Upon our review of your
application, it will be forwarded to the Board of Directors to ensure
membership qualifications and for final approval.
