LETTER OF COMMITMENT
I,
, have had the opportunity to read and
understand the Community Health Improvement Process project charter and I am
committed to the Community Health Improvement Process as a way to improve the
health and wellbeing of individuals and communities in Kittitas County.
I am willing and able to participate in the Community Health Improvement Process as a
member of the Steering Committee which includes attending meetings, participating in
task forces to complete assessment and planning activities, sharing appropriate
resources I have access to, and contributing knowledge and skills relevant to the project.
As part of my participation, I understand that this is to be a collaborative process which
seeks to understand and improve the overall health and wellbeing of Kittitas County
residents.
