DIAGNOTES-REGISTER PROFILE
1. Download the Diagnotes App
2. Open app> enter IUHealth email>
Continue> IUHealth password
3. Select all appropriate Groups>
Next>Subgroups>Next
IUHP Primary Care
DIAGNOTES-REGISTER PROFILE
4. Select Primary Specialty> Next>
Certification> Next
5. Add phone number> Complete
6. Profile setup is complete. The Discussion
page will launch automatically
