The establishment of the institution will be owned and operated by Kristel F. Mission Home of Scholar will endeavor to provide the best suitable learning environment that would allow students access to competent mentoring and adequate tools for the achievement of their highest potential. It is HSA mission to produce independent, responsible, productive members of our ever-evolving society. One (1) Big Brown Envelop/One (1) Big Plastic Envelope ENROLLMENT PROCEDURES Enrollment is the official act of the school in accepting students, provided that they complied with the payments of the required fees and the prescribed rules and regulations for admission. REGISTRATION Students must enroll on the dates designated for the respective year and course. Every student is required to follow the enrollment procedures according to the instructions posted on the bulletin board during the enrollment period. TERMS OF PAYMENT CASH BASIS Full payment of tuition fees during the enrollment period in the Elementary, Junior high school, and Senior high school will be entitled to a 10% discount on tuition only. INSTALLMENT BASIS A required down payment during the enrollment period must be paid while the remaining balance will be equally divided and correspondingly be paid before the scheduled periodic examinations. Permits to take the examinations will be issued by the accounting office upon payment. NOTE: For detailed computations consult our accounting department. REFUNDS OF TUITION AND OTHER FEES A student who transfers or withdraws within two weeks after the beginning of classes must file a written notice to the registrar's office. Refund of fees shall be made in accordance with the following rules. Ten percent (10%) of the total amount due for the term will be charged if he/she withdraws within the first week of classes, or twenty percent (20%) if withdraws within the second week of classes. The student may be charged all the school fees in full if he/she withdraws after the second week of classes. Nonetheless, if the transfer or withdrawal is justifiable, the student shall be charged the pertinent fees only up to and including the last month of attendance (Manual of Regulations for Private Schools, 7th Edition, Page 35). NOTE: A withdrawal letter addressed to the president, signed by the student and parent is a requirement to officially withdraw from school. Release of Credentials: However, credentials or transcripts of records of students will be released only if the student is cleared of any financial obligation. ORIENTATION Freshmen and Transferees are required to attend an orientation program conducted by the Guidance Office upon the opening of classes. SCHOLARSHIP AND GRANTS-IN-AID A. ACADEMIC SCHOLARS With high Honors, With Honors, and With Honors from any school recognized by the Department of Education may apply and avail of scholarship grants provided that the applicant passed the interview & qualifying test and submitted the pertinent document coming from the school attended. Renewable in the succeeding semesters provided that the required grade of 90% is maintained by the student. B. GRANTS-IN-AID SCHOLARSHIPS COMING FROM DIFFERENT SPONSORS IN THE PHILIPPINES AND ABROAD Students with grades not lower than 85% on the report card and who come from financially constrained families may apply and avail of the above-mentioned scholarship provided that the applicants pass the qualifying exams and interview. C. WORKING SCHOLARSHIP GRANTS Available for Senior High School Graduates with grades not lower than 80% and who come from financially constrained families. STUDENT SERVICES In support and contribution to a balanced program of education are the following services. GUIDANCE AND TESTING CENTER Guidance and Testing Center renders services to students and parents. Psychological tests of intelligence, aptitude, personality, interests, and others are available for every student not only for evaluation but for upgrading the students' mental capabilities. Services such as monitoring, career planning, counseling, and others are available upon request. MEDICAL CLINIC The medical clinic is staffed by one full-time nurse and a Part-time Dentist and Physician. Annual Physical check-ups are conducted during the early part of the school year. LIBRARIES The libraries are well-ventilated, spacious, with open shelves and with an adequate collection of professional books, periodicals, and local/foreign reference materials. These facilities are very ideal for research and quiet study. LOST AND FOUND SERVICES Found objects should be surrendered to the Guidance Office. Lost item/s should be claimed before and after class hours only. Items not claimed within a year will be donated to charity. BOOKSTORE The school bookstore sells classroom textbooks, learning materials, school supplies, and other paraphernalia at reasonable prices. CANTEEN SERVICES The school cafeteria is located at the ground floor of the main building. All students are allowed to stay and eat at the cafeteria during break periods only. SECURITY SERVICES The school employs security personnel deployed inside and outside the school premises to maintain orderliness and protect the welfare of students, faculty, and staff. Monitored twenty-four-seven (24/7) by one hundred twenty-three CCTV cameras. STUDENT ACTIVITIES Home of Achievers Academy believes that both co-curricular and extra-curricular activities help students become more well-rounded persons. Every student is therefore encouraged to join at least one club according to their talents and interests. STUDENT BODY OFFICER/ STUDENT BODY COUNCIL The highest governing body of Junior High School and Senior High School aims to facilitate communication among students, teachers, and administrators. Formulate, coordinate, and recommend school activities. GENERAL DIRECTIVES A student who seeks admission to HOME OF ACHIEVERS ACADEMY should be familiar with the school policies. Ignorance of the provisions stated herein does not excuse any student from the corresponding sanctions stipulated. POLICIES Each student of Home of Achievers Academy is expected to abide by the school's policies. These have been created in order that the students can achieve success in a safe and caring environment. It is through these policies that the school imparts positive and desirable behavior, relationships and communication. It is expected, therefore, that each student understands them and adheres to them at all times. 1.ANTI-BULLYING AND HARASSMENT POLICY Definition of Bullying- Bullying is a deliberate hurtful behavior to repeatedly hurt, threaten or embarrass someone. It impacts negatively on the victim’s safety and wellbeing. Forms of Bullying: a. Physical - fighting, pushing, shoving, spitting, tickling, invasion of personal space, or unwelcome physical act. b. Verbal - name-calling, teasing, offensive language, spreading rumors about others, putting people down, making degrading comments about culture, race, physical appearance, gender, religion, or social background. c. Gesture - offensive notes or material, graffiti, or damaging other people's or the school's property. d. Phone Call - harassment through phone calls e. Extortion-stand over tactics, extortion of cast and possessions through threats. f. Exclusion - being left out of activities on purpose with the intention to hurt. Behaviors that denigrate others are prohibited. Most importantly, all members of the school community are responsible for the maintenance of a positive environment. 2. SECURITY PRECAUTION Parents and outside callers are not allowed to see any student or teacher during class hours. They should obtain a permit from the administration office. Cellphone, iPod, PSP, Laptop and other electronic devices that may disturb classes are prohibited during school hours. This is supported by DepEd order #70, S. 1999 and 26 S. 2000 For orderliness and security reasons, the Classroom President and its officers must check the room, and turn off the lights, electric fans, and Air Conditioners at the end of class period. Students are enjoined to take care of their personal belongings. The school will not be held liable for the losses of students' personal property due to students' negligence. No students, parents, or teachers are allowed to transact business in the school unless given permission by proper authorities. Loitering on school grounds after school hours or during weekends especially at night is prohibited. Activities during non-school days or after class hours must secure permits from the administration office duly signed by the Dean/Principal. Conducting meetings of students/school organizations within the school premises without permission from proper authorities is prohibited. Activities outside the school such as field trips, outings, and others, must have written permits from parents/ guardians before securing the approval of the Dean/Principal. No one is allowed to put notices on the bulletin board without proper permission from authorities. Tampering or posting on the bulletin boards without authorization from school authorities is considered a serious offense. For security and easy identification, students are required to wear and display their I.D's. upon entering the school campus. Students without I.D. card or using someone's I.D. shall be subjected to disciplinary action. To minimize problems with outsiders, students are discouraged to bring or tag along outsider friends inside the school compound, near or within the school premises during school or non-school days. Violators shall be subjected to disciplinary action. 3. CONDUCT AND DISCIPLINE All students are encouraged to attend school activities as part of their training formation. Attendance is required during the school flag-raising on Monday. Students are required to stand and sing with utmost respect whenever the national anthem and school hymn are being played. Students must refrain from sitting on tables, running, boisterous talking, laughing, shouting, and whistling on the school premises, classroom, and during social gatherings and activities like programs, dances, etc. It is strictly prohibited to chew gums or any kind of food inside the classroom during classes, convocations, and programs. Littering is also strictly prohibited. Good grooming and personal hygiene must be regularly observed among students. Grooming of hair must be done within the confines of the restroom only. Piercing of the ear/s and wearing of earring/s among male students or double piercing of ears with multiple earrings among female students is strictly prohibited. Tattoos of any kind and distracting dyed hair are also prohibited. Students are expected to obey and show respect to all teachers, non-teaching staff, security guards, utility personnel, and fellow students. 4. UNIFORM All students are expected to wear the uniform properly with pride and dignity. It is, therefore, imperative for students to wear their respective uniforms, according to the prescribed and standard cut, length, and color (see Appendix A for Elementary, Appendix B for Secondary, and Appendix C for College). The school’s official footwear is black shoes and white socks. Rubber shoes are for athletic classes only. Students may only be allowed to wear sandals or slippers during inclement weather only. P.E. uniforms must be worn during P.E. class only. All students must come to school in uniform during school days whether they have classes or not. A complete uniform is required during official functions, like convocations and educational trips; however, the rule may be lifted during certain functions or activities as announced by school authorities. Students are not allowed to wear uniforms for outside activities or for business purposes not related to school activities. They must first secure proper permit from school authorities for valid reasons. 5. LEAVING THE CAMPUS Students are not allowed to leave the campus during school hours. The following precautions are made to protect the students and to minimize the cutting of classes. A student who becomes ill during school hours must be referred to the clinic for an initial check-up. Students who are not fit to attend classes as advised by the physician and will be given a permit to go home. In emergency cases, and upon the parent's or guardian's request, a student will be issued a permit to leave the campus. ACADEMIC REGULATIONS 1. ATTENDANCE Regular and punctual attendance in school is necessary for students to learn effectively and to achieve success in their learning. Absences from classes must be accounted for by a letter of excuse signed by the parent(s) or guardian or physician in case of illness. The letter will be countersigned by the adviser and/or Department Head, and should be attached to the attendance slip to be presented to the subject teacher by the class monitor. In case of Three Unexcused Absences - the parent(s) or guardian will be notified by the adviser and the student will be sent to the Department Head. Cutting of Classes in any subject is equivalent to one half day absence A frequently Tardy Student must present a letter from his/her parent(s) or guardian. Three times tardiness is equivalent to a one-day absence A habitual offender or a student who has more than 10% of absences from the total number of school days (4 to 6 absences/quarter) shall be given a final warning and sent to the Guidance Office for counseling/assessment of the problem. Failure to improve will result in school sanctions as stipulated on page 25 for Attendance Provision and Sanctions. NOTE: Students who dropped or are forced to drop out from the school must secure clearance and dropping-out forms at the Registrar's Office. In case of non-appearance of the teacher in class, the class president must report to the Dean's/Principal's office after 15 minutes of waiting for the latter to take action on the absence of the teacher. 2. EXAMINATION The scholastic year for Senior High School is divided into two semesters: each semester is divided into four grading periods e.g. Prelims, Mid-Terms, Semi-Finals, and Finals For Junior High School, the school year is divided into four grading periods. Each grading period is divided into preliminary and periodic examinations. The schedule of examinations will be posted on the bulletin board a week before the scheduled examination. Students are required to pay their monthly dues before securing an examination permit at the Cashier’s Office. The students who fail to take exams on the scheduled day will not be given special consideration unless his/her reasons have been validated by his/her adviser. No special exams are given after grades for the said grading period have been made. 3. GRADING SYSTEM The averaging system of grading is the method used at SJC. The passing mark in any subject is 75%, the highest grade is 100% and the lowest grade is 65% for junior high, senior high school, and elementary levels. The computation of grades shall be based according to school criteria approved by the DepEd. 4. REPORT CARD/CLASS CARD Elementary and High School Report Cards are given to the parents during ParentsTeachers Meeting that is held after each grading period to clarify matters with the teachers or for consultations. For Senior High School, class cards are issued to the students after the end of each semester. Grades are announced by the Instructors per grading period in order to update/apprise students of their performance ratings. Tampering with the report card/class card is considered a serious offense (see R & R Provisions and Sanctions for detailed information). SCHOLASTIC REGULATIONS CRITERIA FOR HONORS/AWARDS Grade School Pupils with 90% general average and with not lower than 85% in any subjects shall be included in the School's Honor Roll. Achievers must have a general average of 87% to 89.9% with grades not less than 80% grades in any subjects shall be included in the Achiever's list. Meritorious awardees must not have less than 80% grades on the report card, and must have shown exemplary behavior, loyalty and other awards not related to academics. Junior/Senior High School Students with a 90% general average and with grades not lower than 85% grades in any subjects shall be included in the Honor Roll. Junior/Senior High School Achievers must have a general average of 87% to 89.9% with grades not less than 80% grades in any subjects that shall be included in the Achiever's List. Graduation honors for tertiary level as required by SCHOLASTIC DELINQUENCIES a. Junior/Senior High school students with one (1) or two (2) units failing marks shall be re-admitted provided he/she has no delinquency record, and took & passed summer class(es). b. Junior/Senior High school students with more than three (3) units failing marks and college students with more than six (6) units failing grades shall not be readmitted in the following school year. DISCIPLINARY MEASURES AND SANCTIONS The HOME OF ACHIEVERS ACADEMY’s objective is to uphold school discipline at a level consistent with the accomplishment of schoolwork. The school enforces disciplinary measures based on the Manual of Regulations for Private Schools. The guidelines' foremost mission is not to castigate nor degrade the student but to guide and correct the offender to follow the path of right conduct. Penalties imposed on delinquent students will not only correct unacceptable behavior but protect students and the school from corrupting influence of lawbreakers. DISCIPLINARY PROCEDURES The Office of Student Affairs (OSA) supervises the student's behavior. All reported problems of misbehavior are initially investigated by OSA & Guidance Center by ascertaining the facts and imposing disciplinary action/ sanction for minor and major offenses. Students charged with serious offenses shall be investigated and evaluated by OSA/Guidance. Findings and recommendations will be submitted to the Administrative Council for final decision. If found guilty of the offense, the student shall be dismissed. Erring students who have been penalized may appeal for reconsideration of the decision together with parents/ guardians to the School Principal or to the School President. The President's decision is final and irrevocable. At the end of the school year/semester, the record of students shall be reviewed by the OSA/Guidance and Administrative Council to determine if the student with delinquency/ies will be allowed to enroll. DISCIPLINARY MECHANISM AND THEIR CLASSIFICATION FOREWARNING (FW) - a verbal warning to the offender that he/she has a breached the school rules and regulations. WARNING (W) - a written notification to the offender stating his/her offense/s and possible disciplinary action/s. DETENTION (D) - is given to the student who has been forewarned/warned but still committed a minor offense. A detained student shall render service at the school office during the detention period. PROBATION (P)-a written agreement that he/she is under strict disciplinary supervision and therefore must improve his/her behavioral performance within a specific period of time. Breach of the agreement will be subject to dismissal. RETRIBUTION (R)- repayment, replacement, or repair of damaged property. In case of a minor personal offense, a written apology is required to the person concerned. SUSPENSION (SUS)- exclusion from classes and other school activities for a definite period of time. Automatically put under probation within a specific period. A suspended student will be sent to the guidance office to attend a behavioral seminar. COMPULSORY DROPPED -An option given to the parents/ (CD) guardians of a student with academic or behavioral problems who failed to show improvement after monitoring and counseling. FORCED DROPPED (FD) - A penalty given to erring students who cannot cope with the school's academic and/or behavioral policies. EXCLUSION (EXC)- Disqualifying a delinquent student from enrolling in the following semester/year due to overt behavioral performance or academic failures. VIOLATION OFFENSE CATEGORY 1ST 2ND 3RD 4TH 21 Action or Conduct that may discredit the name SUS EXC CD/FD of school 22 Habitual disregard or willful violation SUS P CD/FD established policies and regulations- (May result to expulsion depending on the gravity of offense) 23 Cyberterrorist, cyber extortionist and cyber SUS P CD/FD bullying are prohibited (May result to expulsion depending on the gravity of offense) 24 Violation of RA 101173 and any other offense related to DATA PROCESSING ACT 25 Any other behavioral offense not stated on the list No make-up test, project, and other requirements will be given to the suspended student during the period of suspension. A suspended student must report at the Guidance Office during the suspension period to attend a behavioral therapy seminar. A detained student shall render service at the school office and is not allowed to attend classes during the detention period. In failure to render service, the student will be subjected to disciplinary action. Any offense of the student must be put on record and must be reflected on the Values Education grades. Students with behavioral problems will be evaluated at the end of the semester/school year. Student who failed to show improvement will not be given permit to enroll.
