MEDICATION SHEET
Medication: Lorazepam
Dosage Prescribed:
4mg PO daily,
Classification: Anxiolytics
Is this medication/dosage safe/appropriate for
this client? If not, why? (Be specific)
Tablets: 0.5 mg; 1 mg; 2 mg
Adults: 2 to 6 mg PO daily in
divided doses, with the largest dose
taken before bedtime. Maximum,
10 mg daily.
Actions:
Potentiates the effects of GABA,
depresses the CNS, and suppresses
the spread of seizure activity. Halflife: 10 to 20 hours.
This dosage is safe or appropriate for this patient as it is
within the safe range for this medication.
Nursing Implications:
Monitor hepatic, renal, and
hematopoietic function periodically
in patients receiving repeated or
prolonged therapy. Abrupt
discontinuation or rapid dosage
reduction of benzodiazepines after
continued use may precipitate acute
withdrawal reactions, which can be
life-threatening. To reduce risk of
withdrawal reactions, gradually
taper drug to discontinue or reduce
dosage.
Teachings:
Caution patient or caregiver of patient taking an opioid
with a benzodiazepine, CNS depressant, or alcohol to
seek immediate medical attention for dizziness, lightheadedness, extreme sleepiness, slowed or difficult
breathing, or unresponsiveness. Caution patient that
benzodiazepines, even at recommended doses, increase
risk of abuse, misuse, and addiction, which can lead to
overdose and death, especially when used in
combination with other drugs (opioid analgesics),
alcohol, or illicit substances. Warn patient to avoid
hazardous activities that require alertness or good
coordination until effects of drug are known. Tell
patient to avoid use of alcohol while taking drug. Notify
patient that smoking may decrease drug’s effectiveness.
Side Effects:
CNS: drowsiness,
sedation, amnesia, insomnia, agitation, dizziness,
weakness, unsteadiness,
disorientation, depression, headache, somnolence.
CV: hypotension. EENT: visual disturbances, nasal
congestion. GI: abdominal discomfort, nausea, change
in appetite. Respiratory: respiratory
failure, respiratory depression. Other: pain at
injection site.
Use this form for each of the two psychotropic medications selected and choose different
medication each week for every client selected.
