Introduction
Elec Tricks company recorded a recurring incident of the worker having an electric shock shown
from CCTV footage (Push-Pull) from May 4–31, 2021, involving the worker plugging the vacuum
cord. Upon days of investigation, the safety officers’ finding that there was damaged insulation,
and its wire was exposed, causing the worker to be shocked. With this act, it violates the
company’s safety protocols.
Objectives
The objectives of this safety report were:
1. Identify the primary problem/s of the workplace resulting in an accident.
2. Reduce the risk factor by following the company’s safety protocols.
Incident Information
The worker used a vacuum cleaner, and after he was done, he unplugged the equipment in an
improper way, causing a damaged cord. Later on, the worker was about to clean an area when
he used the same vacuum cleaner, and upon plugging into the socket without checking the
cord, the worker was electrocuted.
Methodology
The safety officer watched the CCTV footage and investigated the scene of accident. The safety
officer also find evidence that caused the worker to be hurt.
Findings
1. The vacuum cord has an exposed wire caused by the worker’s improper pulling of the cord
out of the socket.
2. The exposed wire caused the worker to be electrically shocked upon plugging into the
socket.
Conclusion
In this scenario, the worker was electrocuted by his own wrongdoings. He unplugs the vacuum
cleaner in an improper way, resulting in a damaged cord. Upon plugging the equipment into
the socket, the worker touches the open wire, which then hurts him.
Recommendations
1. Make a habit of checking the equipment’s cord if there’s damage before using it to avoid this
kind of accident.
2. If there’s damage to the equipment, especially the cord, report it immediately.
3. The workers must know and follow all safety protocols, including the proper way of using
equipment.

construction site (Over and Out) from May 4–31, 2021, involving the worker working and riding
in an excavator, close to a power line. Upon days of investigation, the safety officers’ finding
that the worker who was electrocuted did not follow the co-worker’s advice to stop the vehicle
since the excavator was close to a powerline. In this act, it violates the company’s safety
protocols.

Incident Information
The worker on a construction site was riding on an excavator. But then, while working, the
worker’s co-worker noticed that the vehicle was close to a power line and immediately warned
the worker to stop the vehicle. Sadly, he didn’t listen to his co-worker and continued to work.
As the vehicle passes by the powerline, it conducts electricity and grounds the vehicle, causing
it to stop. Later on, the worker steps out of the vehicle without hesitation, which makes him
electrically shocked.
Methodology
The safety officer investigated the scene of accident by interviewing the eyewitnesses to gather exact
details of the accident. The safety officer also find evidence that caused the worker to be hurt.
Findings
1. Because the worker was riding an excavator through the power lines, he was electrocuted.
2. The worker steps out of the vehicle without knowing he might be shocked due to the
vehicle-power line contact.
Conclusion
To conclude, this accident has no one to blame since it is the company’s work to check if the
area of construction is close to a powerline. Also, due to the worker's lack of knowledge about
getting hurt due to the powerline, he was electrocuted.
Recommendations
1. At work the place, especially at the construction site, make sure that the area for working,
driving, parking, or storing materials is closer than 15m (48 ft) to overhead powerlines.
2. If this scenario happens, where a vehicle was grounded by the powerlines, do not leave your
vehicle. If you must leave the vehicle (e.g., vehicle catches on fire), exit by jumping as far as
possible – at least 45 to 60 cm (1.5 to 2 feet). Also, never touch the vehicle and the ground at
the same time.

Incident Information
The mechanic was about to remove the car’s battery but then, upon twisting its lockouts, he
was electrocuted. He uses a crescent wrench tool, but then, when he twists the lockout, it hits
the other parts of the machine, causing the tool to be a path of electrical charges (positive and
negative) and making the mechanic be electrically shocked. Also, the scene gets worse when
he loses his focus and kicks the car, making the car’s cover fall on him.
Methodology
The safety officer watched the CCTV footage and investigated the scene of accident. The safety
officer also find evidence that caused the worker to be hurt.
Findings
1. The mechanic was not wearing a safety glove that’s why he was electrocuted.
2. The mechanic gets angry and loses his senses after the shock.
Conclusion
Although the mechanic was injured electrically by the car machine, in conclusion, it is still his
fault in that accident. He didn’t wear safety gloves and lost his focus, making the accident
even worse.
Recommendations
1. Have presence of mind in doing such work, so that whenever there’s an accident (mild case
or not), it will not cause another accident.
2. Make it a habit to wear personal safety equipment (PPE) to avoid such accidents.

Incident Information
The worker was painting a wall when he noticed his co-worker lying down and being
electrocuted. He wanted to help him and was about to touch his co-worker without even
knowing he would also be electrified. Luckily, a lady warned him not to touch his co-worker and
did the right thing by switching off the power source, then called for help.
Methodology
The safety officer investigated the scene of accident by interviewing the eyewitnesses to gather
exact details of the accident.
Findings
1. The worker doesn’t have enough knowledge and skills to do emergency response.
2. If the worker touches the injured, he will also be electrocuted.
Conclusion
To conclude, a worker must know the basic emergency responses, especially when working in
an electrical field. In order to fulfill that knowledge, a worker must attend seminars and training
to know at least the basics of emergency response so that he/she can help in any accident in
the right way and avoid future or additional accidents.
Recommendations
1. It is okay to immediately help your co-worker when they get shocked, but make sure that you
turn off the power source first before helping to avoid getting hurt also.
2. If there’s a nearby accident, and you are the only one who witnessed it, immediately call for
help.

Incident Information
The worker was about to drill the wall when he noticed that there was damage to the cord. But
then again, he still continues to drill. Upon drilling, it reaches the electrical wires built-in through
the walls, which causes the worker to be shocked and fall. He gets electrocuted two times as
he falls on the damaged cord.

Findings
1. The cord of the drill was damaged, and there was an exposed wire.
2. Drilling electrical wires through the wall caused the worker to be electrically grounded.
3. Due to the exposed wire of the equipment’s cord, the worker was hurt again.
Conclusion
In conclusion, the worker's unsafe actions put him in danger. So, to avoid this kind of accident,
if the cord of equipment is damaged or broken, make sure that you fix it first before using it.
Recommendations
1. Make a habit of checking the equipment’s cord if there’s damage before using it to avoid this
kind of accident.
2. If there’s damage to the equipment, especially the cord, report it immediately, or make a
repair by wrapping it with electrical tape.

due to the unsafe condition of the extension cords connected to the welding machine, accidents
happened, and this violates the company’s safety protocols.

Incident Information
The first accident occurred when the worker was welding steel. He uses an outdoor extension
cord to connect the welding machine. But then, in the workplace, there was a flammable gas
beside the extension, so it caused an explosion. In the second accident, the extension cords are
also involved because the cords are messy, making his co-worker stumble.
Methodology

exact details of the accident. The safety officer also find evidence that caused the worker/coworker to be hurt.
Findings
1. The first incident occurs because of the interaction between the heat extension cord and
the flammable gas. Also, the extension reaches its heat limit because it is working with a
welding machine, which requires more power to function.
2. The second incident occurs because of the disordered extension cords. Since a welding
machine, when connected to an extension, its cord must be spread so that it will lessen the
heat. But then, due to those extension cords, his co-worker stumbles.
Conclusion
The worker needs to acknowledge safety precautions whenever a welding machine is used. In
the end, the worker is definitely the one to blame because his work causes accidents, not once
but twice, and he is not aware of the safety protocol needed.
Recommendations
1. Make a habit to check the workplace if there’s a thing that can danger everyone especially
when you’re about to work with a welding machine.
2. If a worker uses an extension cord to connect a welding machine, make sure that the cords
are scattered in organized so that they cannot harm other people by stumbling them.

In this occurrence, it is the worker’s fault for having a habit of improper installation of a lightbulb
that’s why he was grounded. Furthermore, he did not wear safety gloves, making the situation
dangerous.
Recommendations
1. If necessary, switch off the power source when installing a lightbulb for safety purposes.
2. Do not be afraid to ask for assistance, even if the task is as simple as replacing a light bulb,
especially if the installation is in a high location.
3. It is best to wear safety gloves to avoid being grounded.

A machine in a factory must be inspected daily, and each worker has their own inspection time.
Upon checking, the first worker inspects the machine’s electricity or power supply. But then,
the second worker came, intending to check the machine parts inside. However, he appears to
have forgotten that each worker has a set amount of time to complete their task, which is why
he was locked out of the machine while it was running.

To conclude, the second worker was hurt because of his actions. As a worker, you must be extra
cautious in following safety protocols so that bad situations, such as being trapped inside a
running machine, do not occur. like being locked inside a running machine.
Recommendations
1. Always be attentive in doing such work.
2. In this scenario, if you want to finish your task even if it’s not yet your time, just inform your
co-workers that you will begin working/inspecting.

Incident Information
The worker was drilling a wall in a public toilet, and he used a long extension cord. Thus, the
cable blocks the doorway, causing damage to the insulation and exposing its wire. Later on, a
man enters the restroom and washes his hands. But then the hand dryer didn’t work, so, as a
result, the man was electrically shocked when he was about to leave and touched the door
since his hands were still wet.
Methodology

exact details of the accident. The safety officer also find evidence that caused the worker/coworker to be hurt.
Findings
1. The cord was damaged and has an exposed wire done by continuously opening and closing
the restroom door.
2. The man's wet hands grounded him as he exited the restroom and touched the door
because it was connected to the exposed wire of the cord and now has electrical charges.
Conclusion
The man was hurt because of the unsafe act of the worker. He did not realize what was going
to happen to that extension cord since it blocks the restroom door.
Recommendations
1. If there’s no other available socket inside a restroom, you can use an extension cord. But
make sure that the cord does not block the doorway to prevent damaging the cord.
2. Use a blocker on the door and inform people that the place is under construction.
