Intro: ● Hello I’m Blank I’ll be your nurse for today, can you confirm your name and date of birth for me. Ok thank you. Perform hand hygiene, provide privacy, verify providers orders. So mr. Johnson, I’m going to be performing tracheostomy suctioning, and tracheostomy care. This is to keep your airway clean and your stoma hygienic and help with the risk of infection. Do I have your consent? And do you have any questions? ● Raise bed to hip level, I’m going to put you in semi-fowler's position and auscultate your lung sounds. There are some adventitious sounds therefore suctioning will be beneficial. Put the barrier down on pt. Part 1 ● Increase O2 hyperoxigenate verbalize to pt. Perform hand hygiene. DON PPE FACE MASK and GOWN ● OPEN SUCTION KIT POUR SALINE INTO BASIN (outside basin) ● DON STERILE GLOVES ● COIL catheter in DOM hand Part 2 ● Turn on suction to 120 mm hg ● Connect to suction catheter ● Dips tip of catheter into basin to flush and moisten ● Non Dom hand remove trach mask Part 3 ● Insert with dominant hand until resistance is met ● Withdraw suction ½ inch & apply suction intermittently rotate catheter as you pull out, (each suction 10-15) ● RINSE CATHETER AND REPEAT (reapply oxygen between suction) ● DC suction catheter, turn off suction, discard supplies CARE ● ● ● ● Part 2 ● ● ● ● ● Part 3 ● ● ● W/ Same sterile gloves discard stoma dressing ASSESS COCA Remove sterile gloves Perform hand hygiene Open trach care kit (place drape) (pour saline into basin) Don sterile gloves Pre-moisten gauze and cotton tips Non dom hand remove trach mask and unlock cannula then remove Insert new inner cannula and make sure its locked and secure Using cotton tips clean trach site using circular motion from in to out Use gauze pads to clean outer cannula and stoma site, pat dry w/ dry gauze Stabilize trach w/ non dom hand and place fenestrated gauze under the cannula Replace trach ties w/ old one already on and then remove others (YOU ARE NON STERILE HERE) Part 4 ● ● ● ● Discard everything Perform hand hygiene Auscultate lungs and return O2 to normal level Raise side rails, bed locked and lowest position call light, hygiene
