RE: A REPORT ON AN ILLEGAL CONTRACTOR THAT ENTERED OUR
NETWORK WITHOUT PERMISSION
1. INTODUCTION
With reference to the letter dated 3rd October, 2012, with Reg No – 273/GLBU/D1/ / 2012, from
Gwagwalada Business Unit. We were directed to investigate an illegal contractor that entered our
network without permission at Maikwari Village in Kwari Area Council. We investigated that the
job started since 1st September, 2012, before the accident scene which happened on Friday being
21st September, 2012 by 10.00am.
2. MEETING WITH THE BM
On our arrival at Gwagwalada business unit, we met the SMD Gwagwalada business unit, he
represented Business Manager, who was absent as of the time of our arrival.
The SMD said he is aware of the accident, and also acknowledged the receipt of the said letter
from the Kwali Area Council through the Assistant Service Engineer on Monday 24th September,
2012, The letter dated that the accident happened on 21st September,2012, around 10.00am but got
to the Assistant Service Engineer by 2.00pm of that day of the incident. This delayed the
submission of the letter to the business unit on that same Friday, but it was submitted on Monday
of the following week being 24th September, 2012.
After the interaction with the SMD, he advice us to meet the Assistant Service Manager of Kwari
Area Council, who was to be around on that day of the accident.
3. MEETING WITH THE SERVICE ENGINEER
We proceeded to Kwali Area Council; here we met Mr Victor Igboegwu, who is the Assistant
Service Engineer, (He was on duty on the day of the accident).
Mr Victor Igboegwu said that on Friday 21st September, 2012 by 10.00am, somebody called him,
and told him that there are people working on PHCN 33KV line, erecting poles without approval.
On the process, the three men got electrocuted and one died instantly. The other two were taken to
an unknown hospital. When he received the information; he went there to verify what really
happened. On reaching there, he found out that the villagers had already called the police. The hiab
driver with the registration number BWR 298 XA had been arrested, the deceased had been
removed and other victims taken to an unknown hospital.
On the day of the accident, around 2.00pm, the Kwali Area Council brought in the notification of
award of contract for the extension of electricity and installation of 500KVA Transformers dated
1st of September, 2012, a Saturday, non working day. He said that he got the letter through a
PHCN marketing Staff who was discharging his normal official duty at the area. The PHCN
Marketing Staff said that the letter was given to him by one man in that area, who pleaded with
him to help him and submit the letter to his office and he collected the letter but handed it over to
the Assistant Service Manager.
On knowing the content of the letter Mr Victor Igboegwu called the service Manage at Abaji (Mr
Usman) and told him about the letter and its content, the SMD as well, who advised hm to write to
the Business unit, which he did.
4. VISIT TO THE SITE OF ACCIDENT
Furthermore, we proceeded to the venue of the Accident at maikwari village. We saw the pole on
the ground .From our interaction with the villagers and even the chief’ Son, we learnt the project is
going on irrespective of what is happened.
Photographs of the accident scene and the network as the time of our visit are also attached for
your investigation.
5. INTERFERENCE FROM THE AREA COUNCIL’S LETTER
We went back to the Kwali Service Centre, there we were told that letter from the area council was
back dated to Saturday 1st of September, 2012, a non working day.
A copy of this letter is attached for your perusal.
