OCCUPATIONAL SAFETY AND HEALTH ONLINE COURSE
GUIDELINES and CONSENT FORM
I.
BEFORE THE TRAINING STARTS
a. Participants who have shown interest in attending the course are required to
accomplish the Online Registration form provided by our marketing team via
email. Answer and upload all necessary files needed (school ID/valid ID, 2x2
picture, signed guidelines and consent form) to complete your registration
including this form that you have acknowledged. A meeting ID and Password
shall be provided after registration.
b. Only those you have submitted the Online Registration form is considered
confirmed for that specific course schedule.
c. We will be using ZOOM as our platform for this online course, It is required that
participants download the application in their laptop or desktop PC. If you are a
first-time user YOUTUBE has a lot of videos that can help you be familiar with
the application.
d. It is a requirement of OSHC (Occupational Safety and Health Center) and
DOLE that participants use a LAPTOP or DESKTOP PC and have a stable
internet connection to avoid experiencing video, audio and connectivity
problems during the course. The following are the system requirements for
ZOOM:
1. Broadband wired or wireless (3G or 4G/LTE). Minimum bandwidth is
600kbps (up/down) and recommended is 1.5 Mbps (up/down).
2. A microphone – built-in to your computer or USB plug-in or wireless
Bluetooth.
3. Built-in headset jack from laptop or desktop. Even if your laptop or desktop
has speakers,
you must plug in a headset so that the sound from the speakers does not
cause an echo in the Zoom meeting. Any headset with a 3.5mm plug will plug
in.
4. A webcam - one which is built-in to your computer or one that connects to
your computer via USB.
5. Supported Operating Systems that are current: Mac OS X with Mac OS
10.6.8 /(Snow Leopard) or later, Windows 10, Windows 8 or 8.1 or Windows
7.
e. Wherever you might choose to attend the training, prepare a place with ample
lighting, low ambient or background noise and a decent background (you can
use virtual background function from zoom) for your video profile.
II.
DURING THE TRAINING
a. Upon entering the zoom meeting, you will be placed in the waiting room. This gives
the Host time to verify if you are registered for the course. You shall be admitted
once your name is confirmed. Upon joining the zoom meeting, you are required to
use your COMPLETE NAME as your zoom profile name. Unrecognized names
shall not be admitted. Kindly include your school/university acronym before your
complete name (ex. UST – Juan Dela Cruz).

b. Before the start of the training, as previously discussed in item (e) of the first part,
we suggested that you choose a place with ample lighting and decent background
for your video profile. This is for the purpose that we are randomly taking snapshots
of the zoom meeting as a requirement of DOLE and OSHC for your attendance.
c. Even if this is an online course, we are required to be seen decent in our video
profiles. Please avoid wearing sando, tank top or sleeveless shirts during the
session.
d. When interacting with other participants verbally or via chat box we are expected to
be respectful and professional with everyone.
e. As a requirement of DOLE and OSHC, Participants are expected to be seen
visually attending the course. We are bound by their CATCH UP POLICY. Once
you are not seen visually (video turned off) and mostly disconnected from the
zoom meeting for more than 10% of the time allotted in a module, you will not
receive your CERTIFICATE OF COMPLETION unless you attend a MAKEUP
CLASS on another schedule.
f.
This online course is also covered by the DATA PRIVACY ACT. Your personal
Information and video recordings are all collected as a requirement of DOLE and
OSHC for attendance verification review and record purposes of this prescribed
course. Video recording, taking of pictures and screen captures during the training
by a participant is NOT ALLOWED without consent.
g. The host, staff and moderators shall be in constant communication with the
participants during the session. You will be asked to start your video (if seen turned
off without reason) or change your name. If we do not receive any response or
reaction for a certain amount of time the participant shall be placed in the waiting
room.
III.
AFTER THE TRAINING
a. Video recording and snapshots of the session will be reviewed before certificates are
released to verify your complete attendance.
b. Certificates will be available 7 calendar days after the concluded session via email.
The Host Company has the right to remove any participant who does not comply with the rules
set by this DOLE Prescribed course.
DATA PRIVACY CONSENT
In Compliance with the Data Privacy Act of 2012, I allow Ovaldesk Inc. and AMOSH to:
1. Share my information to affiliates and necessary third parties for any legitimate business
purpose. I am assured that Ovaldesk and AMOSH are employed to protect my
information.
2. Retain and continue to use my information in Ovaldesk Inc’s Database shared with
AMOSH.
I also acknowledge that I have read and understand the guidelines set and mentioned above for
this online course.
