Cognitive Development:
> He reads very slow and has little comprehension.
> He has very limited sight words.
> He has short attention span and easily gets
distracted.

> He often stutters when speaking.
> He uses incomplete words and
ungrammatical phrasing.
> He is unable to summarize or tell a story in

Self- Care Development:
 He does not brush his teeth regularly
because he often forgets to do so.
 He sometimes forget to put back the
things he used.
 He did not take a bath everyday and
sometimes did not change his clothes
regularly.
AFFECTIVE DOMAIN:
He is curious and explore the environment.
He plays with adults ( cousins) and peers.
He initiates interaction through calling.
CHILD’S SKILLS:
He can able to spell words like cat and two.

FINDINGS
Dashy is found to be having difficulties
and delays in terms of his Cognitive ,
( learning difficulties )

and even his Self-Care or his adaptive behavior

The Child manifested the characteristics of a learner who
has developmental delay such as:
>Late talking ( He is already 5 years old that he
learned to talk or speak. He has limited vocabulary.
>Difficulty in reading long words.
> Difficulty in answering basic WH- questions.
> Difficulty in remembering the sequence of events.
> Having hard time in caring himself like changing his
clothes regularly.
RECOMMENDATIONS:
The child needs constant guidance and supervision of the
parents.
Talk to him a lot.
Continue to have social interaction.
The child showed / manifested the characteristics may be
developmentally delayed and should be observed further.
He also be given formal assessment or speech language
assessment to address his problem in communication.
