AFFIDAVIT OF UNDERTAKING

of the Philippine National Police (PNP), after having been duly sworn to in accordance with law, do
hereby depose and state to wit:
That, I was appointed into the PNP service on ____________ with General Order Number

That, I have never been convicted of criminal or administrative/disciplinary offense and has no
pending case either administrative/disciplinary/criminal offense in any investigative body/tribunal or
court, and I am not a witness to any ongoing criminal or administrative case, especially those covered
by the provisions of RA 9165; and I am not a summary hearing officer with unresolved case;
That, I have not violated any provisions of The International Human Rights Law or
International Humanitarian Law;
That I am not presently facing any attrition proceedings;
That I have not applied for optional retirement and I am still on active duty for the next two (2)
years or more;
That I agree the nullification of my travel and I hold myself liable for perjury, falsehood,
misinterpretation, or omission of facts;
That I shall abide by the PNP Code of Ethics and Professional Conduct and I shall never
commit any act/s or omission/s that will cause dishonor or disgrace to the PNP;
That I will strive to achieve outstanding accomplishment in the performance of assigned tasks
in order to bring honor and prestige to the Philippines;
That I attest to the truthfulness of the foregoing statements and submit to the legal and
administrative consequences thereof if ever the statement above are wanting in truth and in
substance; and
That I am executing this affidavit freely and voluntarily.
