1. Document your focused musculoskeletal assessment of Edith Jacobson.
 I asked Mrs. Jacobson if she was having pain she mentioned, a 2/10 pain range. I
assessed the patient’s pain level and found that the patient had tenderness in the
her left hip area. I later, assessed her legs for strength, and mobility. Her left leg
was shorter than her right leg. There was an even color skin tone throughout. The
patient had good strength in her right leg but her left leg was weakened. Mobility
was detected in her right leg, and left leg was weakened.
2. Document your focused skin assessment of Edith Jacobson.
a. I assessed, the skin of the legs for color, temperature and texture. She has normal
elasticity of the skin throughout on both sides of her legs. The skin had normal
coloring corelating with her elasticity. The legs showed no signs of moisture or
wetness.
3. Document the patient teaching that Edith Jacobson would need related to fall risk and
prevention during her hospitalization and upon discharge.
a. I educated the patient about fall risks, and I made sure the call bell was in great
reach. I reminded the patient that’s he was at high risk for fall due to her
medications. I told her daughter, not to move Ms. Edith on her own, and informed
that a nurse would reposition her every 2 hrs.
4. Document the essential elements of proper body alignment for Edith Jacobson and
include the specific supplies that would be needed.
a. I mentioned to Mrs. Edith that good alignment was needed to have great recovery.
I informed her of the constant reposting that would be done for her, by the
physicians orders. I made sure the call bell was in reach, I called in a second nurse
to with repositioning to prevent additional injury to the patient. I used the slide
sheet to help roll the patient on her right side. I made sure that the patient is in a
comfortable position while on their side.
Opening Questions
1. How did the simulated experience of Edith Jacobson’s case make you feel? It made me feel
great knowing that it was my first time actually doing an assessment on an elderly women. I had
a great experience with taking care of my patient and learned a lot about the drugs she was
taking, as well as the risks she was in, pertaining to falls.
2. Talk about what went well in the scenario. For this scenario, I was able to properly reposition
my patient with her falling on showing any signs of pressure ulcers. My patient showed no signs
of infection, and her amount of pain was at a 2. Her vital sign were stable and she is breathing
great on her own.
Reflecting on Edith Jacobson’s case, were there actions that you would do differently if you were
to repeat this scenario? If so, how would your patient care change? If I had to repeating this
scenario I wouldn’t of had placed her sitting up, as I did in the beginning. I noticed, that she
screamed for pain, and mentioned I was hurting her, I made sure to never do that again. This
would of caused damage to her hip and place pressure on her lower extremities. This was very
uncomfortable. My care would have changed by making sure my patient was comfortable.
What feelings were you experiencing during your interaction with Edith Jacobson? I felt bad that
she was in the hospital because of her hip. I feel that many older people fall and are at risk for
injuries of the hip which can put them in danger. I know that her daughter is very scared because
her mom is in the hospital and she is much older. Many times, I feel like the older you get the
weaker your body is and many complications can occur.
Scenario Analysis Questions
T&C Discuss the role of teamwork related to Edith Jacobson’s care. The role of team work was
to reposition the patient. I knew that I couldn’t do it alone so I had to call in another nurse
for assistance. By doing thing it made matter easier when turning the patient to her side.
Having proper body mechanics and being able to move her successfully was a plus.
Without team work it wouldn’t have been properly, and Mrs. Jacobson could of has
injury to her legs.
T&C List some ways that teamwork and collaboration can be improved. Discuss how you
would implement your suggestions. Teamwork and collaboration could have been
improved by having the other nurse counted to 3, signaling the transition. Instead, the
screen displayed a movement. I feel that counting to 3 would have made a safer approach
to the transition, because both sides would have been ready.

Describe your role in ensuring Edith Jacobson’s safety. I want to make sure that during
her stay she doesn’t not catch any hospital infection. I want to make sure that Ms. Edith
doesn’t fall on her side, instead use a call bell for help. I want to make sure her vital signs
are stable and that she is able to be cooperative with repositioning every 2 hrs. I want to
make sure that her pain level remains lower that 2.

Discuss two evidence based practices that you incorporated into your care of Edith
Jacobson and how these practices impact patient outcomes. One evidence based practice
was repositioning the patient every 2 hrs, to reduce the signs f pressure ulcers. Know that
she is much older, at 85 its very easy for her to get ulcers because she is on bed rest. So, I
want to follow orders and reposition her so that she doesn’t get any pressure ulcers, and
that her body is receiving great oxygen and nutrition to regenerate. The second , practice
was calling for assistance when I could not turn the paint on my own. If I did not do this I
could have broken my back, or put the patient at more risk for injury, while trying to turn
her alone.
Concluding Questions
Describe how you would apply the knowledge and skills that you obtained in Edith Jacobson’s
case to an actual patient care situation. In an actual patient setting I know now to never sit a
patient up after they have received hip surgery. This will cause pressure down below. Also, I
would always call in another nurse if I need help repositioning my patient, because great body
alignment when lifting a patient is important.
