business address at #36 JP Rizal Street Barangay Sta. Lucia Novaliches Quezon City, do hereby
name constitute and appoint Ms. Michelle Maambong, of legal age, Filipino with business
address at #99 Jose Abad Santos Street Barangay Sta. Lucia Novaliches Quezon City, Philippines
to be my true and lawful attorney in-fact to act, for and in my stead, to do and perform the
following acts deeds and things, to wit:
1. To act as my representative before the Bureau of Internal Revenue in all transactions relative
involving transactions for application, process and release of collection receipts.
2. To process, sign, receive or claim all documents in relation to collection receipt application
due the Company that Bureau of Internal Revenue will issue henceforth.
3. To do such any other act or thing that may be required necessary or incidental to carry out
Effectively and for the purpose of collection receipt application for which this authority is
hereby given;
HEREBY GIVING AND GRANTING unto my said attorney-in-fact full power and authority to do
and perform any and every act, and thing whatsoever requisite and necessary to be done in
compliance to the regulations of the governing agency of ________________, as fully to all
