Case: Violation of R.A 9262 Violence Against Women and their Children
Purpose of testimony: Your honor the purpose is to offer the testimony of the witness and to
prove the following based on the material allegation in the criminal information:
a. That the Witness and the accused are legally married.
b. That at the time of their marriage the accused displayed irrational behavior and failed to
support the child
c. That the Witness will testify on other matters.

2. Do you remember that you executed an affidavit?
3. Can you affirm the contents of the affidavit?

4. What is your relationship with the accused?
5. According to the affidavit, your husband displayed irrational behavior right?
6. How long have you been married?
7. Can you give proof that you are legally married to the accused?
8. Can you confirm that the marriage certificate attached marked as “Exhibit B” is your
marriage certificate?
9. Can you describe your relationship with your husband during these eight long years?
10. Do you have kids with the accused?
11. How old is your child now?
12. Do you have any proof that the father of your child is the accused?
13. Can you confirm the birth certificate attached marked as “Exhibit C” is your child and
that the signature signed is from the accused?
14. What’s the occupation of the accused?
15. Did he regularly send support to your child?
16. What are the things you did in order to get financial support?
17. Can you confirm the attached “Exhibit D” is a letter you sent PNP chief to request
an allotment to your child?
18. How did he react about the letter?
19. After the letter was sent, what was the result?
20. Can you confirm that the attached “Exhibit E” is the memorandum of agreement
(moa), you and your husband agreed upon?
21. What was the moa all about?
22. Did your husband fulfilled the stipulations in the moa?
23. Did he send you money after the moa?
