the Inamotan Elementary School, Manaoag, Pangasinan effective April 26, 2021 after the expiration of
my Maternity Leave with pay on April 25, 2021.
I was granted one hundred five(105) days Maternity Leave with pay from January 10, 2021 to
April 25, 2021.
Attached herewith is my Medical Certificate duly signed by Medical Officer that I am now fit to
work.
