 BACKGROUND
◦ A worker’s lost 3 fingers when operating injection
molding machine.
◦ DOSH interfered the investigation, company engineers
started up investigations and report writing.
 ROOT CAUSE
◦ Human behaviour, poor supervisory performance, loose
rule enforcement, long working hours.
◦ Worker tends to skip precautionary step, remove hand
once placed pellets.
 ROOT CAUSE
◦ Place pellet into machine and press 1 button only.
 CORRECTIVE ACTIONS
◦ Management control – enforce strict rules, tighter
supervision, mandatory interwork breaks
◦ Wear PPE
◦ Engineering control – built in proximity sensors, 2
buttons- make sure both hands are out of platform.
HAZARD IDENTIFICATION– INJECTION MOLDING PROCESS
 RISK ASSESSMENT- WORKERS’ HAND GOT CLAMPED
 RISK CONTROL –

◦ ELIMINATION-Ban injection molding (impossible)
◦ SUBSTITUTION – consider alternative process or
automated injection molding machines
◦ ENGINEERING CONTROL – creatively modify the injection
molding machine with extra safety
features
◦ MANAGEMENT CONTROL- impose strict SOP and tight
supervision
◦ PPE – Provide relevant PPE for workers
