RESPONSIBILITY
5.1
The applicant shall:
5.1.1 Fill in the PTW Form and display the form at the work area.
5.2
QA Executive shall:
5.2.1 Keep a copy of PTW Form for documentation.

PROCEDURE
6.1
GENERAL
6.1.1
PTW issuance to the vendors/suppliers/subcontractor’s is only meant for all highrisk tasks that covers these following activities:

PTW shall applicable for the vendors/suppliers/subcontractor’s that perform high
risk activities as stipulated above.

7.0
PROCEDURE
6.2.1
The applicant shall fill in the PTW Form (refer Attachment A) completely and
signed by the applicant for the work including the ATP’s Person of Contact.
6.2.2
The applicant shall submit a complete PTW Form to QA Department. QA shall
distribute the complete form to AIROD’s Security and Safety Department by email
for their copies.
6.2.3
A copy of PTW shall be kept by QA Department for reference.
6.2.4
PTW shall be displayed at the work area for employee references by the applicant.
6.2.5
The applicant shall ensure that the work area is clean and safe.
6.2.6
Once the task is completed, the applicant and the ATP’s Person of Contact shall
sign the PTW and returned it to QA Department for documentation.

PENALTY FOR FAILURE TO COMPLY

Fine not exceed fifty thousand ringgit (RM 5,000.00) or to imprisonment for a term not
exceeding six (6) years or to both.
