To enroll your child as a pa ent at a school health center, he or she must be enrolled as a student in the school. A registra on form must be completed by the child’s parent or guardian before your child may be seen at the school health center. Registra on forms are available at the school office, in the school health centers, and at Fam‐ ily Health Center of Worcester loca ons. Services Available at School Health Centers Family Health Center of Worcester and the Worcester Public Schools have been partners in the school health centers for over 20 years. The school health centers offer a comprehen‐ sive array of preven ve and primary care services in the loca on where children spend most of their me: in the school. The school health center provides students with easy access to quality health care. It is open during regular school hours. Health care is provided by a licensed nurse prac oner working in conjunc on with physicians and other resources from Family Health Center. “The best thing about school health centers is that we take care of the children where they are. Students and families are not billed for medical services provided at the school health center. If you have health insurance, including MassHealth, please complete the insurance informa on sec on on the registra on form so we may bill the insurance company directly. This support helps to cover our costs. Will the School Health Center Work with my Child’s Primary Care Provider? Yes, we will work with both your child’s health insurance plan and Primary Care Provider. We will also coordinate with specialists as needed.
