Cancellations/Changes/Refunds:
Cancellations made by May 6, 2016 will receive a refund minus a $30.00 processing fee. Any cancellation made after May
6, 2016 will not receive a refund. All refunds will be processed after the conference. Substitutions are allowed at no
charge. Note: Hotel cancellations must be made 24 hours in advance in order to avoid a $75.00 cancellation fee.

Boat Cruise:
Because many attendees will be arriving on Sunday, May 15th in preparation for the convention kickoff on Monday
morning, we have organized a boat cruise on the Milwaukee River and Lake Michigan for Sunday evening. The cruise will
include appetizers and drinks. This event is free for conference attendees, however space is limited. To reserve your spot
on the boat cruise, please indicate below if you would like to participate.
For those who are not interested or unable to join the boat cruise, we will also have a cocktail reception overlooking Lake
Michigan at the Potawatomi Hotel & Casino.

Hotel Cancellations:
Hotel cancellations must be made 24 hours in advance. Cancellations made less than 24 hours in advance will be subject
to a cancellation fee of $75.00.
Shuttle Service:
Potawatomi Hotel & Casino will also offer periodic shuttle service from Milwaukee’s General Mitchell Airport. To take
advantage of this service, please send your flight information to sales@reportexec.com so we can plan for your arrival.

Potawatomi Hotel & Casino is a privately owned and operated business, therefore:

Management has the right to refuse service to any guest and may request a valid ID at any time while a guest is
on the premises.
•
No weapons are allowed.
•
Smoking is not allowed in designated non-smoking areas, including all restaurants.
•
No outside food or beverages may be brought into the facilities and no beverage containers holding alcohol may
leave the casino (special dietary needs/special occasion carry-ins require written authorization by Casino Senior
Management in advance).
•
Appropriate clothing, including shoes and shirt, is required to enter the premises.
•
Bags, backpacks, purses and luggage are subject to search.
•
Individuals must be at least 21 years old to access the first floor of the Casino, restaurants and theater
performances.
•
Guests must be at least 18 years old to play bingo. Guests must be at least 18 years old to access third-floor or
Hotel restaurants, unless accompanied by an adult (over 21).
•
No faces may be concealed while on property.
