Comply with all requirements of Section 10 2513 PATIENT HEADWALLS AND 01 3300 SUBMITTAL PROCEDURES. It is understood that this is a preliminary MOCK-UP coordination submission for initial review of the ICU Room mock up and further submissions may be required to insure adequate coordination and address any mock up comments. Comply with all review comments from 10.29.2015 25th Street Mock Up review (notes inserted in submittal by TCCo, dated 11.11.2015) including any follow up emails and subsequent clarifications / client directions and as noted in this submittal review. 2. Provide complete LEED submittal for review in accordance with Part 1.5 of the specification. 3. The client has requested that the dialysis box be located as high above the finish floor as possible. Ideally the top of the stainless steel enclosure / cover flange would be 1” minimum below the equipment rail. Please confirm this is sufficient room for installation and construction / fabrication tolerance for laminate clad panel assembly. The 31.5” dimension from headwall centerline to centerline of the dialysis box is approved. The dialysis box location and mounting heights are approved as submitted. TCCo submit proposed alternate latching mechanism for review with client / coordinate installation of dialysis box with alternate latching mechanism (non-keyed) as directed by NYU. 4. The client has confirmed that the initial mock up is to be an ICU headwall assembly. 5. Nurse Call back box to be installed as noted in this submittal for the initial ICU mock up review. Please note that the nurse call location and requirements are still under review by the client. Submittal Review 07.22.2015 01.05.2016 Call system device locations as shown are approved. Coordination cuts were only provided for the Single Patient Station and Bed Receptacle; I have inserted a cut of the “354018 – 2 Jack Station” identified as “AX” on the shop drawings, please insure appropriate coordination has occurred with associated subcontractor. Electrical device colors currently under review by client to insure conformance with current hospital standards. Color and device design style to be reconfirmed under separate cover. Device colors as indicated in this submittal were approved as client standard for this project. Vertical location of electrical devices is acceptable as shown, horizontal location needs to be coordinated with final AV equipment system components and monitor selection. Metal laminate type and grain direction is confirmed and was previously approved by NBBJ. Please note attached SKA-151 for Patient Bedroom 100 on floors 8 through 18, the south side of the headwall assembly has been furred out in order to resolve a beam / riser conflict reviewed as in BIM coordination process, coordinate with TCCo as required. Final laminate TBD, laminates to be reviewed and confirmed with client once installed at the 25th Street Mock Up. PL-01 Wilsonart #10776-60 “Kensington Maple” (with laminate cross grain printed to allow for vertical grain with no vertical seams) is approved. Anodized aluminum finish previously approved. The only detail reviewed or approved under submittal “0003-102513-0 Headwall & Footwall Trim Details” was the custom perimeter trim extrusion. Previously requested section details should be provided prior to final fabrication. TCCo Note: EEG face plate and device types to be confirmed / coordinated with equipment vendor. Cover plate finish to be brushed stainless steel to match adjacent devices. Contract documents call for entire area of recess to receive blocking (sheet metal backing plates), this also needs to be coordinated with final equipment selection and placement. Coordinate with note 7. Footwall not submitted as part of this package. Four Acute Physio Monitors need to be provided on 18th floor at Patient Bedrooms 112, 113, 114 & 115 (please note these room numbers have been updated by ASI 138 to rooms 13, 14, 15 & 16 respectively.) Confirm all headwall types and configurations are in accordance with contract documents, coordinate updated room numbering with ASI 138 as required. As reviewed in the 25th Street Mock Up, accept the concept of eliminating the previous “pocket” concept to allow for removal of the headwall face panels. I do not understand why you would want to incorporate the painted aluminum “L” shape above the headwall panel, why not just extend the GWB ceiling plane to intersect the headwall partition assembly (above the finished removable face panel). The light fixture is installed in a simple cut-out in the GWB ceiling, the fixture location needs to be coordinated with headwall installation clearances, then reviewed and coordinated with associated carpentry subcontractor. MEP systems / pathways of the mock up installation as follows: 0 a. Device / Outlet Alignment: All devices to be aligned horizontally and vertically on their centerlines. Currently the medical air or vacuum outlets (by device manufacturer) are not centered on their cover plate (fabricated by Modular). Modular to adjust installation as required to insure devices are centered on their cover plates and installed in the headwall with all device and outlet centerlines aligned. (Photos 1 & 2) Photo 1 Photo 2 b. Vacuum Piping Installation: While the overall piping installation is in accordance with previous review comments, the current 90⁰ piping offset at the connection to outlet is not acceptable. Modular to modify the piping to utilize 45⁰ piping connectors. (Photos 3) Action By Modular Status Item Notes Action By Status Photo 3 c. Emergency Power Junction Box: There was concern expressed that the there is insufficient space in the current compartmented junction box to insure ease access to all of the required wiring connections. Modular is to increase the width of the back box by 2 1/2” to 3” and increase the depth of the box by approximately 1” if possible. The increase in width is achievable however Turner and Modular will need to confirm id increase in depth is achievable given constraints of adjacent bade building pippin risers for the dialysis box, etc. (Photo 4) Photo 4 11/11/2015 Page 2 of 7 Item Notes Action By Status d. Piping / Dissimilar Metal Isolation: Overall concern was expressed that all dissimilar metal to metal conditions are properly isolated, that the isolation mesh is of sufficient length and that the mesh is properly secured to insure it will not sag or slim over the headwalls serviceable life. Modular is to review all conditions to insure the length are sufficient (Photo 5) and that all areas are properly isolated (Photos 6). Photo 5 Photo 6 e. Pipe Clamps / Supports / O-Ring Isolation: The current use of the “Proflow” type of pipe clamps is acceptable to Facilities, as long as they are adequately secured to the adjacent framing. Modular is to provide individual round cut outs in the steel framing, with the addition of the plastic O-ring type gaskets in lieu of current larger rectangular un-gasketed openings. (Photos 7 & 8) Photo 7 11/11/2015 Photo 8 Page 3 of 7 Item Notes f. Action By Status Dialysis Box Drain / Piping Pathways: Dialysis box installation was not complete nor was it reviewed as part of the mock-up observations. Dialysis box shall have a right hinged gasketed, lockable door. Modular and Turner to confirm that waste line / trap and water supplies for dialysis box have been sufficiently coordinated with electrical conduits to insure proper clearances and pathways have been maintained. Modular and Turner to also confirm that the dialysis box plumbing services, riser and or lateral runs of the piping, have been fully coordinated: framing openings properly sized, sufficient cavity space, etc. have been provided, and piping support and installation methods incorporated.(Photos 9 & 10) Revised per JBB comment 11/2/15. Photo 9 Photo 10 g. Aluminum Perimeter Trim: Concern was expressed about some minor flexing or looseness of perimeter aluminum trim. Modular to review the tolerance on the “shelving standard” type connectors utilized to secure the vertical aluminum trim as well horizontal base trim. An additional screw will need to be added at the top of each vertical to insure they remain tightly secured over time. (Photos 11 & 12) 11/11/2015 Page 4 of 7 Item Notes Action By Photo 11 Status Photo 12 2. 1The following design /aesthetic comments were noted, please review .and respond as required: 1 Modular a. Headwall Horizontal Equipment Rails: There appears to be a bit of play in some locations between the back of the fixed horizontal rails and the face of the laminate panels. The rails need to be fabricated to allow easy removal of the individual panels, however Modular is to review current tolerances to insure a consistent joint where the two materials intersect. In general the overall aesthetic appearance of the headwall is very good. (Photo 13) Photo 13 11/11/2015 Page 5 of 7 Item Notes Action By Status b. Footwall Horizontal Laminate Butt Joints: The current laminate to laminate butt joint is not acceptable as it is not a flush condition. This may have to do with how the unit was shipped to get into the 25th Street mock up, however Modular to review and confirm type of joint to be used to unsure a flush condition. The footwall should be treated like a full height wall panel as it will not need to be demounted like the headwall. (Photo 14) Modular also to confirm if it is possible to achieve a consistent laminate “wood grain” match vertically. Photo 13 c. Perimeter Trim and Interior Trim at Footwall: The mitered corners in the trim are not flush or uniform. Modular to confirm the joint type (Butt joint of miter) that will insure the most uniform and consistent connection between the horizontal and vertical trims. (Photos 14 & 15) Modular and Turner need to also confirm the tolerance required where the footwall trim meets the adjacent GWB wall surface. Modular to confirm how the trim is secured to the footwall given it does not need to be demountable. 11/11/2015 Page 6 of 7 Item Notes Action By Photo 14 Status Photo 15 This documents our understanding of the issues discussed. If any participant takes exception to the information noted above please inform the writer. Submitted: 11/11/2015 10.30.2015 John C. Flanagan, AIA Page 7 of 7 Turner & Modular Services Company comments dated 11/17/15. 1A. John Flanagan asked if we can change the med gas trim rings so that the gas outlets are centered vertically. There is more stainless steel exposed on the bottom. See attached photo. Turner – This will not be a problem, all devices will be centered on their cover plates, and all device/outlet centerlines will all line up. MSC – Confirmed, we will tighten up the tolerance for the clearance opening in the trim plate and center the cut out. MSC will send out replacements for the mockup for review and approval. 1B. Danny Bensimon is requesting that we use two 45’s on vacuum line offsets. Turner – This will not be a problem. MSC – See below backup attachments for a picture of two 45’s connected to a ½” pigtail. 1C. Paul Natter asked if the large junction box located in the bottom, center of the headwall be increased in size. Turner – The no fly zone for the dialysis box risers were heavily coordinated, and cannot be adjusted more than ½” wider, and then 2” lower. This would ensure there are no hits with the already drawn/coordinated field piping in 3D. MSC – The junction box will be increased by ½” w and extended 2” lower to create a larger junction box compartment. Aside from this item, it is our understanding that the pull boxes, junction boxes, conduit, conduit connectors, conduit clamps, and wire were all approved as is. 1D. Danny Bensimon is requesting that the yellow mesh that we apply to the copper for dissimilar metals be longer and secured in place (possibly taped). The copper is also touching the horizontal bracing down by the outlet so we need to add the mesh in this location as well. Turner – This will not be a problem. MSC – See below backup attachments, we will protect and/or tape on in requested areas by using 3M #472 type tape which is listed for separation of dissimilar metals. 1E. Change large cut outs of the studs for the med gas lines to individual round openings. MSC – See page #7 of Supplemental File document which shows our preferred method of using the circle clearance opening in our steel vertical supports along with the black plastic bushing to protect against dissimilar metals as the primary function. If we need to add the proflow clamps for additional support we can look at this. 1F. Danny Bensimon asked if the conduit that crosses directly beneath the Dialysis box is in the way of the drain or feed lines. Turner – This has been coordinated, and there will be no clashes. MSC – Please see below backup attachments for dimensional data, there will be adequate space. 1G. Add an additional fastener to the top of the side trim, and all tolerances adjusted and tightened. Turner – This will not be a problem. MSC – Confirmed. 2A. Gap between the panel and the equipment rail. Turner – This will not be a problem. MSC – Confirmed, we will provide the necessary shims to push the panel towards the face of the extrusion. 2C. Perimeter & Interior trim at footwall. Turner – Modular still reviewing footwall design and if additional fasteners would be required for interior trim. MSC – The footwall design and scope of installation is being reviewed separately. MSC confirms that the inner trims can be changed to have a “flat” joint (non-mitered). The horizontal joint will butt up to the vertical due to the trim design. 2B. Butt joint quality & lining up wood grain lines up above and below the field seams. Turner – Modular still reviewing footwall design, but since the footwall does not need to be demountable, all joints whether they be vertical or horizontal, can utilize splines/biscuits & glue to ensure butt joints align. Expansion/contraction of wood due to humidity at butt joint design is still a concern over time. MSC – Initially it was thought to be possible though hard to guarantee through large costs for material waste & labor efforts, but then after speaking with Wilsonart, they confirmed there is variation from sheet to sheet in the laminate when the material is spliced together at their factory because of roll sizes and running it cross grain. 3 Vinyl Tape 472 Technical Data Product Description 3M™ Vinyl Tape 472 is backed with a pigmented rubber adhesive for abrasion protection and many outdoor weathering applications. Each console outlet consists of a rough-in assembly and a frontbody assembly. Rough-In Assembly The rough-in assembly is available with a copper inlet tube (piped backbody) or a DISS male inlet fitting (DISS backbody) for connection to a gas delivery line. The piped rough-in assembly includes a 7” (18cm) long, type K copper, inlet tube for brazing to the gas delivery line. This inlet tube rotates 360°, which permits alignment with the gas line. Both the piped and DISS rough-in assemblies consist of a steel backplate with a non-removable, positive pin-keying arrangement for each specific gas service. The specific gas service is stamped into the front of the roughin. Both types of rough-in assemblies include a fully serviceable secondary check (for pressurized gases) to help prevent gas flow when the frontbody is removed for service. Vacuum and WAGD services do not have secondary check valves, according to NFPA 99 standards. Frontbody Assembly The console frontbody consists of a DISS keying disc and attached barrel assembly, which houses the primary check valve. The keying disc assembly includes a gas-specific, positive pin-keying device, is color-coded, and labeled with the name of the gas service. The barrel assembly includes brass piston and frontbody check spring, which engage and seal when a DISS-fitted treatment device is removed from the outlet. Features • Quick access to primary check valve components. • Large, gas-specific, color-coded, keying disc and label. • Outlets listed by Underwriters Laboratories® and CSA® and manufactured in accordance with applicable CGA, NFPA 99 and CSA® standards. • Cleaned for oxygen service, pressure-tested, and capped prior to shipping. We are indicating that the laminate color for the upper and lower device mounting panels to be Wilsonart #7909-60 “Fusion Maple”. Please verify that this is what is required. Correct, except finish was changed to “78” (Fine Grain) in email correspondence mentioned above. We are indicating a horizontal grain direction for this wood grain selection per Bid Clarification #2. Please verify that this is correct or if the material should be ordered cross grain for a vertical orientation. Wood grain to have vertical grain direction - NBBJ Please note that the accessory channels and ½” reveals will have a clear anodized finish. 10. The architectural elevations show the devices right next to one another. Based on the spacing indicated on the drawings, we are currently indicating quad face plates as we provided in the initial mock-up for these devices. Please verify that this is what is required or indicate if single gang plates are preferred. Quad cover OK - NBBJ Please note that if single gang devices are preferred, MSC will space the plates out appropriately to provide enough clearance for the device openings in the panel. 11. Please verify if the receptacle at 24” AFF behind the bed should be engraved "BED MOTOR ONLY" to help prevent other devices from being inadvertently plugged into the receptacle. Do not engrave receptacle – JBB/NBBJ 12. It is our understanding that the device face plates should be engraved with the panel board/circuit #. Please verify that this is correct and complete the provided MSC Engraving Schedule. TCCO to provide schedule – Chris Brooten 13. Initial information received indicated that only the two normal power devices directly next to the dialysis box require GFI type receptacles. Please verify that this is correct or indicate if other devices on the dialysis side need to have GFI type receptacles or none of the devices have this requirement. GFI type receptacles are not required - JBB 14. It is our understanding that the ICU units are to include a monitor channel only that mounts directly to the unit with internal bracing to support the monitor and that the tilt/swivel bracket to support the monitor will be provided and installed by others. We are indicating that the channel is to be a standard GCX type 13" length channel. Please verify that this is what is required. Yes, pending owner review. – NBBJ 15. For the data outlet provisions, we are indicating a compartment with a two gang opening along with a conduit from this compartment to the ceiling line for routing wiring from the ceiling space above (wiring to the receptacle is to be provided by others). Please verify that this is what is required. Wiring and faceplate will be furnished by the IT contractor – JBB/NBBJ. 16. Currently, we are not showing any switches on the headwall design to match information previously received and Architectural elevations. However, the lighting plans indicate (4) switches (two on each side of the patient) to be installed on the units. The switches are shown to be (3) single pole type and (1) 3-way type switch. Please verify if these switches should be added into the design and indicate the desired location on the unit. No, they are outside of headwall – NBBJ/JBB If these switches are required, please indicate the voltage (120 or 277) and the circuit type (normal or emergency) feeding the switches. Switches are not a part of headwall – NBBJ/JBB 17. MSC will provide the appropriate clearance openings within the framing system for others to route the vent (2”), waste (2”) and water line (3/4” + insulation). Please verify that the size of each line shown is correct. Confirmed – JBB/NBBJ. MSC has been coordinating the clearance openings in the assembly with Turner through BIM process – see enclosed MEP drawings. 18. The framing system for the headwall will be designed to accept the attachment of finishes on the Footwall side except for the (23) rooms highlighted in the room list on the enclosed drawings. These rooms will have separate modules to attach the panel kits as required. Please verify that the wall construction in these rooms will be typical with (2) layers of gypsum on the face of these partitions. Room 115: 2 layers. Room 500: 1 layer - NBBJ 19. Please provide details on the TV or mounting bracket being used on the Footwall so that MSC can provide the appropriate bracing. Bracket is Chief Mfg, model LSMU.
