Pre-Start Up Checklist

Please fill out this form and return it to Vemco 48 hours prior to desired start up date.
Start up will not be scheduled until a completed checklist is returned.
DELAYS
IF, WHEN ARRIVING AT THE JOBSITE OUR SUPPORT TECHNICIAN IS
DELAYED IN STARTING EQUIPMENT OR HAS TO RETURN TO THE JOB DUE TO
ANY OF THE ITEMS BELOW BEING INCOMPLETE/FALISFIED OR, FOR ANY
REASON NOT THE RESPONSIBILITY OF VEMCO, INC., EXPENSES INCURRED
WILL BE CHARGED BACK TO THE REQUESTER OF START UP AT A RATE OF
$750.00 / DAY PLUS MILEAGE AT $1.00 / MILE (FROM PORT TO PORT)
