What Is a Wellness Program?
ESNJ is committed to the health and wellness of our team members. The primary
objective of the ESNJ Wellness Program is to enhance the quality of life of our team
members. In partnership with BHS, this wellness program is designed to provide
employees with the necessary resources to improve their well-being and become more
engaged in every aspect of their health.
ELIGIBILITY
All employees enrolled in the medical plan may participate in the
wellness program.
New hires must wait until the start of the next wellness cycle to
participate in the Wellness Program.

SURCHARGE
The wellness cycle will run from January 18–December 31, 2016.
The non-wellness participant fee will end May, 2017.
All employees who do not complete the two program requirements
(annual physical and Health Risk Assessment [HRA]) will be
charged a $25 per pay Non-Wellness Participant Fee in addition
to their employee contribution to the health insurance premium.
The surcharge will begin with the pay dated August 8, 2016 and will
continue through the end of the wellness cycle of 2016/2017.

2. Click on MyBHS Portal.
3. Type username: ESNJ
4. Under the “Quicklinks," click “Secure
Health Portal.” First time users will
need to “Register Now” by entering
Company: ESNJ, member ID number
(six digit employee ID number), date of
birth, gender and email address.
5. Once registered, participants may take,
complete or view their HRA, email
their coaches, view and register for
upcoming activities, access resources
such as articles, tips, FAQs on a variety
of health and wellness topics, get program announcements and more!

Program Requirements
1. Complete an annual physical with your primary care physician (PCP) and submit
the Annual Physical Form to BHS by June 30, 2016. Annual physicals completed
between June 1, 2015 and June 30, 2016 will be accepted.
2.Complete an HRA online or over the phone by June 30,2016.

The Foundation for Wellness | ONLINE OR TELEPHONIC HRA
An online or telephonic HRA is available to all participants. All results from the HRA are confidential and are for educational
and awareness purposes only. Receiving and understanding results will serve as the foundation on which participants can build a
healthier and happier life.
Tools for Change | UNLIMITED WELL-BEING COACHING AND CONSULTATION
Unlimited coaching will be available to all program participants. Well-Being Coaches will develop a personalized action plan based
on the participant’s HRA and biometric screening data, current lifestyle behaviors, health needs, areas of concern and personal
wellness goals. Well-Being Coaches answer general wellness questions, provide preventive health and complementary care
information, establish short- and long-term goals, provide ongoing encouragement and motivation and assist participants in locating
appropriate health and wellness resources in the following areas: Complementary care (e.g., yoga, acupuncture, chiropractic),
building healthy relationships, career development, emotional well-being, fitness and exercise, healthy eating and nutrition, healthy
lifestyles, reducing disease risk factors, stress management, tobacco cessation, weight loss/management, work/life balance and

Mental Health and Wellness | EMPLOYEE ASSISTANCE PROGRAM (EAP)
BHS EAP services are a free company-sponsored benefit that provide you with an easily accessible, safe and confidential means
to help with personal or professional problems that may interfere with work or family responsibilities. You can receive up to three
sessions per person, per issue, per year. Services include:
• Short-term face-to-face/telephonic problem resolution
• Legal and financial consultation services and childcare and eldercare referral services
• Referral services, liaison with community resources including health insurance carriers
• Access to a panel of EAP professionals nationwide—24/7, 365 days a year
