Supplier Instructions for Preparation of Supplier Deviation Request (SDR)
Section B - Supplier Responsible to Complete Information
The supplier shall use the SDR form to submit technical and quality deviations from a Request for
Proposal/Quotation (RFP) or Purchase Order (PO) requirements. Resolution shall be accepted by Southern
California Edison (SCE) prior to performing services or shipping material to San Onofre Nuclear Generating
Station (SONGS). The supplier shall complete an SDR for EACH deviation from the RFP/PO requirements to
permit the prompt resolution of the various deviations by allowing each to be individually dispositioned. The
supplier is responsible for the completeness and accuracy of all information in SDR Section B and all
attachments. All attachments shall be referenced on the SDR. Number all pages consecutively with the SDR as
page one (1). Please print, using black ink, or type.

Indicate the deviation type by checking the appropriate block. Deviation types are explained below with
clarifying examples and attachment requirements. Reference to part numbers should be interpreted to include
other relevant nomenclature necessary to maintain configuration control.
Nomenclature Change Only (Box 1): part number(s) cited by SCE has been changed and does not
involve a change in form, fit, function, or material of part(s). The part must be identical in all aspects
(dimensions, operation, construction, mounting and internal parts) except nomenclature. Complete and
attach a Form 26-468, Page 1 only.
Item With Configuration Change (Box 2): recommended deviation does involve a change in form,
fit, function, or material of part(s). Complete and attach a Form 26-468 Page 2. The Supplier must
provide SCE with a complete description of the physical, performance, reliability, functional, code,
standard, qualification, drawing and manual differences associated with the substitute item. It is
recommended that any information which will assist SCE in assessing the acceptability of the substitute
part be provided with the SDR to ensure a prompt resolution. Explain fully if the substitution is an
alternate for the original part or if special circumstances warrant a one-time substitution.
Other Exceptions/Deviations/Discrepancies to RFP/PO Requirements (Box 3): describe the
condition on an attachment. Reference the attachment in the supplier's recommended disposition section
on the SDR. Ensure exception / deviation / discrepancies do NOT conflict with SCE’s commitments to
codes and standards.
If the Form 26-468 does not provide an adequate format, you may use an alternate document of your own
design providing that ALL information requested on the respective forms is included in your SDR attachment.
The SDR must reference all pertinent attachments which support your substitution recommendation.
Identify the SDR number(s) of any previously submitted SDR(s) that pertain to the subject of this SDR in the
space provided, or state "NONE."
Provide signature/title/date of authorized supplier technical representative in the spaces provided. Mail this
form and all attachments to the Southern California Edison procurement agent.
When the approved SDR is returned by SCE, review the disposition and take appropriate action.
NOTE:
The Substitution Evaluation is an SCE internal evaluation document which determines the
acceptability of a configuration change and does not require supplier review.
