Intake Petition Form
This form must be submitted to the Greek Life Office prior to the commencement of intake activities. It is the
responsibility of the Greek Life office to check the grades of all prospective members before they are extended an
invitation to be a part of any NPHC or MGC organization line. Please complete the form with names of the candidates for
membership intake. Only candidates that are enrolled at Baylor University during the semester in which intake is
scheduled to occur are eligible for membership intake. The candidates must have an earned GPA of at least 2.67 and 12
semester hours at Baylor. This form will be kept confidential and will be shared only with proper university officials.
