Subject: Report of no-cost travel
Hi Steve,
Just wanted to notify the Ethics Board that I just returned from a no-cost to the City trip to Alexandria,
VA to attend Public Health Accreditation Board (PHAB) Site Visitor training. This travel was approved by
my immediate supervisor (Deputy Commissioner Jaime Dircksen), the Commissioner of CDPH and OBM.
PHAB paid for all travel related expenses including meals, hotel, airfare and ground transportation,
totaling $1122 (this is only an estimate as I never received a bill for the hotel--it was prepaid, so I
estimated the hotel cost based on online hotel prices). Please let me know if you require additional
information, or have questions.
