I’ve returned from Indianapolis for the Open Forum for Quality Improvement in Public Health from
4/6/16-4/8/16. The Public Health Accreditation Board covered all expenses ($1440.19) for this travel
which included airfare, hotel, registration, meals and ground transportation. Please let me know if you
