Progressive discipline, termination of employment
Dear xx:
This letter is to acknowledge our conversation today regarding your employment with
the University of Missouri. As of today, you are being discharged from your position as
(title) with MU Extension (dept). Specifically, this termination is a result of (i.e.
continued poor performance, continued absentee problems, etc).
Per University policy HR-601, employees may be discharged for incidents which are
serious enough to warrant summary discharge, or after less severe disciplinary actions
have been taken.
You were given an oral warning on xx/xx/xxxx, a written warning on xx/xx/xxxx, and a
suspension on xx/xx/xxxx. [if applicable] You were given a performance improvement
plan on xx/xx/xxxx AND/OR it was discussed at your performance evaluation on
xx/xx/xxxx in which you were to implement goals towards improving your performance.
Unfortunately you have continued to demonstrate performance issues that include:

Pursuant to policies of the University of Missouri,
 You will be paid lump sum the value of your unused vacation days.

 You will be removed from payroll the last day of actual work. Medical and dental
benefit eligibility continues until the end of the month of separation.
 You may contact the Faculty and Staff benefits office at 573-882-2146 to obtain
information about your non-vested contributions, your vested retirement,
and/or your potential COBRA benefits.
 Please verify that your home address is correct on myHR at
https://myhr.umsystem.edu. This is the address that the University will use for
future contact.

You may be contacted by Extension HR to conduct an exit interview. If not, you
may request to complete an Exit Survey with me, my supervisor or human
resources.
