This form is for use in the Salary Review for non-clinical professors, who are Head of a
Department or School, to request a review of his or her band in the Professorial Pay Scheme.
Professors are asked to complete this form, filling in details of notable achievements and
contributions drawn from the last five years under any or all of the headings below, drawing

Research
Please provide a summary evidencing your most significant research achievements (e.g.
recent publications, research grants/contracts won, research projects managed, lecture series,
visiting research positions, external assessment, conference invitations, membership of major
research/grant awarding bodies, editorships, prizes/awards/honours etc.).

Please provide a summary evidencing your most significant teaching achievements (e.g. QAA
return, curriculum development, visiting teaching positions, authorship of standard texts, PhD
supervision, external examining, and membership of major teaching/grant awarding bodies,
prizes/awards/honours).
Leadership & Management
Please provide a summary evidencing your most significant achievements in leadership and
management (e.g. leading a research group, guidance and management of staff, research
projects and contracts; setting up and driving forward innovative research or pedagogies;
initiating and driving major initiatives or partnerships; acting as chief adviser nationally;
managing change, e.g. restructuring).
