Principal Investigator Statement
As the Principal Investigator, I am responsible for the management of sponsored projects funds and effort
expended on the sponsored project. The completion of the Assignment of Proxy form does not remove that
responsibility as the Steward of the sponsored project.
The Proxy identified below has first-hand direct knowledge of the employees work and funds allocated to
compensate the employees performing work on the sponsored project.
Principal Investigator Attestation
By signing the Assignment of Proxy I am confirming the Proxy has first-hand direct knowledge of the employees
work and compensation paid on my sponsored projects. For any sponsored project that the Proxy does not have
first-hand, direct knowledge of the work performed, I will be responsible for reviewing those effort reports.
By signing the Assignment of Proxy, I give permission for the Proxy to certify and review the effort reports
associated with my sponsored projects.
