Senior Case Aide Purpose Purpose: The purpose of this position is to assist the New Connections Program. Essential Functions Function 1: Prepares and maintains chronological notes, clinic charts, or case histories for each client. 12 % Time Function 2: Provides guidance as to best means of assisting clients to resolve program issues, makes agency appointments, and offers referrals to community social service agencies. 12 % Time Function 3: Maintains client follow-up by telephone to determine whether clients' needs have been met; Makes appointments for home visits. 12 % Time Function 4: Prepares daily, weekly, or monthly statistical activity reports as required. 12 % Time Function 5: Participates in staff conferences and clinical staffings. 12 % Time Function 6: Tracks service authorizations and billable services provided for the Staying Connected program. 12 % Time Function 7: Monitors and records services to children for the New Connections Prevention for Drug-Exposed Children program. 12 % Time Function 8: Provides prevention services for children/families on a substitute/assistant basis. 16 % Time Marginal/Incidental Functions Marginal/Incidental Functions: Other functions as assigned. Qualifications Required Qualifications: High school graduation or GED equivalent. 1 year of experience in social services, direct services, or administrative support services.
