I Call to Order 5:30pm – Charlie Tveit The meeting was called to order by Chair Charlie Tveit at 5:35pm. Charlie provided an opportunity for public comment. II Welcome and Introductions III Business 5:40pm – Charlie Tveit Jake Greer made a motion to approve the meeting minutes of the March 20, 2014 meeting minutes as presented. Ray Simms seconded the motion. The motion was presented and passed unanimously. 5:35pm – Charlie Tveit Tule proposed that the CAC/CHIP 2 meetings in North Lake. Following discussion, the group agreed to organize 1-2 meetings in North Lake County. Charlie updated the committee that hospital reimbursements for medicaid clients have changed to a values based model for many of the hospital's in Oregon, however, as a Critical Access Hospital, Lake District Hospital will remain on a cost-based reimbursements plan. John and Charlie attended the RCAC regional implementation strategy worksession and provided an update: The group established Guiding Principles for a regional implementation strategy: To be successful, we have to raise funds, our work is critical – together we have greater collective impact. We will recognize unique local needs and projects, which we regionally promote and support, but the locals are responsible for acquisition of funds. We identified who would be the fiscal sponsor or fundee organization: a. Short-term – Oregon Rural Health Quality Network (ORHQN ) act as fiscal agent b. Long-term – create 501c3 – GOBHI will contribute lawyer to complete task (in-kind) We will ask for funding based on what we need (and not less). Identify if the project is scalable. If so, then we can receive less funding. If not, then we can't accept funding for less than what it costs to do the project (prudent/appropriate budget). Allocation of funding is based on data-driven needs assessment with a baseline based on need. We will look at a collection of data across all counties and allow for cost of delivery. We discussed who would be in charge of writing proposals. We want one person to be the main contact person and coordinator of developing proposals. Proposal development will be a team effort, with counties and lead organizations also contributing. Next step is to present to the RCAC at end of April. Also, we will reconvene the group in early May to develop an event proposal. IV Oregon Health System Transformation and Community Health Improvement 5:45pm – Maikia Moua Maikia Moua, who is a Transformation Analyst with the Transformation Center, a program of the Oregon Health Authority, discussed Oregon Health System Transformation and provided the latest updates about what's happening statewide. She provided a power point presentation. IV Lake County Community Health Improvement Plan 6:10pm – John Adams John Adams presented the revised Lake County Community Health Improvement Plan, with updated measurable outcomes for several of the strategies. Sherry Chamberlain seconded the motion. The motion was presented and approved. VI Lake County CHIP Year 2 6:20pm – Justin Valley/John Justin Valley presented on the strategic planning process for year 2 of the CHIP program. The intention is to determine if other issues need to be addressed, identify other strategies, identify other potential long-term strategies, and develop a 3 year strategic planning document for health improvement for the county. The goal is to have the plan complete by the fall (September?) regional fundraising event. Timeline includes: May - brainstorming session June community meetings, which will provide further community input and shape the strategic planning process 1st workgroup meeting in late June 2 workgroup meetings in July 2 workgroup meetings in August September – finalize the strategic plan/document Paul McGinnis and Estella Gomez offered to share other implementation ideas that are happening around the region and state. By June, all 12 CCO CHIP plans will be released. John Adams discussed the June community meetings. The Lakeview meeting is scheduled for June 19th, at the Elks Lodge, 5:30-7:30pm. The North Lake meeting will be scheduled at a later time. (It has been scheduled for Thursday, June 5th, at the North Lake Clinic, 6:00-8:00pm). Each committee member has been asked to invite 5 people to the community meeting and to return to the May meeting with the names of the people who said yes.
