Upon the recommendation of the Search Committee, and the Dean of the (COLLEGE NAME), I am pleased to recommend
to the Chancellor and the President that you be appointed Chair of the Department of (DEPT) and Professor of (DISCIPLINE), with
tenure, beginning on (DATE). 1 This offer is subject to the approval of the President and Trustees of Indiana University. In your
case, we have every reason to believe that such action will be forthcoming. Employment in this position is contingent upon a
satisfactory background check. The initial salary for this position is ($ AMOUNT) for the fiscal year and as a new employee of IPFW
you will be paid by Purdue University. In addition to your salary, Purdue provides a generous benefit package that includes a
defined contribution retirement plan as well as medical, disability and life insurance which will begin immediately upon your

initial duties will consist of (%) administration, (%) research and (%) teaching with the expectation that you will also teach one
summer class on-load, along with normally expected university and professional service. You will receive a separate mailing about
the New Faculty Institute and you are expected to attend.
A comprehensive review of your service as department chair will take place in the spring of (YEAR) 3. In the event that
you no longer serve as chair, your appointment would change to an academic year basis by the standard conversion.
Enclosed you will find President's Office Form 19 "Purdue University Appointment to the Faculty" and "Notice of Terms
of Initial Appointment for Appointments to Indiana University Missions." To indicate your acceptance of the appointment, please
sign these forms, as well as this letter, and return them to me in the envelope provided. I will then forward them for action and
approval by the President and Trustees. The enclosed Employee Information, Self Identification Compliance, Background Check
Form, Direct Deposit Authorization, W4 and WH4 forms should also be completed and returned at this time. Please refer to Indiana
University's Academic Handbook, which can be found online at
https://www.indiana.edu/~vpfaa/academichandbook/index.php/Main_Page on the web. It details policies and procedures that apply
to IPFW faculty appointed to Indiana University missions. Also for your reference, Purdue University’s Faculty and Staff
Handbook, which can be found online at http://www.purdue.edu/faculty_staff_handbook/. In addition, I am sending you University
policy I.A.1 and copies of Purdue executive memoranda (B4, B48 and B50) and OAA 05-3 which are relevant to your employment.
The policy and memoranda are for your permanent records.
Employment with the University is subject to the requirements of the Immigration Reform and Control Act (IRCA) which
requires employers to verify an employee’s right to work in the United States. Prior to your first day of work, you must complete
Section 1 of the U.S. Citizenship and Immigration Services’ Form I-9, Employment Eligibility Verification. To access the electronic
form, visit the IPFW Human Resources site at http://new.ipfw.edu/hr/. Click the link titled, “Electronic I-9 for new hires/rehires,”
login and complete Section 1 of the Form I-9. You will then be provided a list of acceptable documents. The required documents
must be presented to your school’s business manager or Human Resources on or before your first day of employment.

For offers contingent upon VISA classifications, remove “with tenure” and add the following wording: “, contingent upon proper visa classification.
Also note that tenure cannot be awarded to a faculty member until he or she has received permanent residency.”

For offers with moving allowance, insert the following language “In order to assist you with your move to IPFW, you will be provided a moving
allowance of ($ AMOUNT). This lump-sum payment represents taxable income and taxes will be withheld from the payment. If these funds are used
