Directions: Please read the following instructions before filling out this form. Choose your first preference of 4
courses from the provided list of offerings and write them below. Next, write 6 alternate choices in case your first
4 preferences are unable to be fulfilled. An example (Ex.) is provided on the first line of the form. To maintain J-1
visa status, undergraduate exchange students must take 12 credit hours (4 courses) for fall and spring semesters.
Occasionally, an exchange student with an excellent academic record will want to take 15 credit hours (5 courses).
Approval of the Director of the School of Business International Program is required in order to do so.
Please note: This is a fill in form. Please type your information, print the form and send it in. To better your
chances at receiving your first preference of courses, please submit this form no later than October 15.

Marketing (MKTG), and Production (PROD). A short description of each course and the required prerequisites are
also provided under the link above. If you are requesting a course outside the School of Business, please be sure
you have completed the prerequisites.
