MANUAL: Health Science Clinical Work - Based
TRANSPORTATION:
It is the policy of The Anderson 5 Career Campus that a student who is enrolled in the
Health Science Clinical Work - Based class shall provide their own transportation to and
from clinical sites. Students are not to be dependent upon other students for transportation
to clinical sites or to home schools.
The student will provide evidence of automobile insurance. The company name and
policy number must be provided to the clinical instructor.
The student will provide evidence of health insurance. If the student does not have health
insurance they must purchase insurance through the school. The name of the company
and policy number must be provided to the instructor.
Emergency contact names and numbers must be provided by all students to the clinical
instructor.
The automobile, health insurance and emergency contact information should be kept with
the clinical instructor all times in the event of an accident.
