Counseling Program Assessment: Spring 2013 Review
Please complete this form for every counseling program area.
Summarize only what has already been completed as of the end of Spring 2013. On the last page of this form, we
will ask about your tentative plans for future activities. (Submit what you have now. There's no requirement to
cram in additional work at the end of the semester BEFORE completing this form.)

* Be sure the SLOs you the same ones that are posted on your website.
Helpful Hints:
1. ERROR MESSAGE when backing up to review entries?
If you back up to review a page while entering data, you might end up with an error message saying you can't
proceed without completing the answers to all questions (which, of course, you've completed). If you encounter
this error message, just back up all the way to the first page, and then move forward again, page by page, until
you get to where you left off. This should solve the problem.
2. What are our editing options?
After you choose SUBMIT, as long as you keep your browser window open, you can go back and change your
answers by choosing the "edit submission" link. Also, since a password is now required, and you login with your
CCSFMail (Google) account (@mail.ccsf.edu), you will get a copy of your entry through email. You can follow the
link in that email to return to your entry at any time and edit it again. Also, as with past versions of the form the
most recent submission of a given course or service area is the one that will appear in the online data review.
3. To speed up entry time, please make sure you have everything you need with you (SLOs, data, meeting
notes, etc.) before you begin entering data into the form.
