El Camino Community College Safety and Health Committee Wednesday, May 28, 2014 Present: Adrian Amjadi, Robert Brobst, Rodrick McMillan and Valerie Wagner (Recorder). Absent: Rocky Bonura, Janice Caldwell, Elizabeth Fernandez, Julienne Gard, Christine Killduff, Ana Tontcheva and Carl Turano. A. Call to Order: Valerie Wagner called the meeting to order at 1:15 pm 1. 2. Minutes: Minutes of January 22, 2014 were approved. Workers’ Compensation: Valerie Wagner reported that for the month of April, there was a total of seven injuries and a total of thirteen days away from work. 3. Office of Safety and Health: Valerie Wagner reported that she is in the process of scheduling training for employees that have a valid AED/CPR certification card on how to properly check the additional eleven AED Units that were recently installed in the Campus Theatre, Facilities Break Room, Humanities, Library, Life Sciences, MBA Building, Social Sciences Building and the Student Services Center. Valerie Wagner also report that approval status is still pending with regard to our Hazardous Materials Business Plan by the Los Angeles County Fire Department Inspector, Stan Uyehara. 4. Safety-related Work Order Report: Robert Brobst reported the following concerning work order numbers: 58836 – Awning needed over the cashier’s windows which face south at Bookstore Building. This item does not have any type of resolution at this time. 60191 – Deep fryer in Deli at the Art Patio is leaking oil at all times. The Deep Fryer has been Red Tagged. 60916 – Install a lock on the two sliding glass doors of the Green Room Balcony and roof access door in front of the Green Room will not lock. This work order is in progress. 61023 – The Auxiliary lights on the southwest stairwells on all three floors of the Humanities Building were off. This work order has been completed. 61297 – Exterior Light fixture outside of PE South locker Rooms is loose and needs to be secured to ceiling. This work order has been completed. 61086 – Stall divider and door are falling apart in Men’s Public Restroom located on the 1st Floor of the Music Building. This work order is still in progress. 61184 – Repair several light fixtures in the Mechanical Room of the Natural Science Building. This work order is in progress. 61188 – Repair ceiling light cover/fixture in Social Sciences, room 120. This work order has been completed. 61478 – Remove and clean-up raw sewage from Life Sciences restrooms near planting area. This work order is in progress. 61495 – Instructor locked himself out of his office in the MBA Building, Room 233. This work order has been completed. 61550 – Replace missing fire extinguishers from cabinets on 3rd and 4th Floors of Parking Lot “H”. This work order has been completed. 61641 – Repair broken steps at the entrance of the Construction Technology. This work order is in progress due to the roadway closure because of the construction. 61657 – Fill large pothole on the exit road near the Child Development Center. Our Grounds Department has put in a temporary patch but we hope to have a permanent fix after July 1st. 61705 –Door to Art & Behavioral Sciences needs to be modified. If door is locked from the inside then there SAFETY & HEALTH COMMITTEE Minutes of Wednesday, May 28, 2014 Page 2 of 3 would be no way to unlock with a key from the outside. This work order is in progress. 61812 – Removal of hardware from beneath computer tables in Humanities, Room, 122. This work order is in progress at this time. 61822 – Ceiling tile is loose and about to fall in Industry and Technology Division Office. Carpenter could not find the loose tile. 61851 – Repair kickboard on one student’s work stations on the 1st floor of Technical Arts. This work order is in progress. 61873 – Please check for smell of a natural gas odor on the exterior, west side of the Communications Building. Michael Blada tested this area with the gas meter but findings were inclusive; gas meter did not detect any gas odor. 61886 – Door to Announcer’s booth on Baseball Field will not lock. This work order has been completed. 5. Old Business: Art Department – Adrian Amjadi reported that the chronic problem with both the doors being left unlocked and lights being left on overnight has been resolved. Robert Brobst commented that the custodial staff runs were recently changed and now these issues are coming up in a different part of campus. Adrian Amjadi also reported the problems with Domestic Linen have been resolved. 6. Division Reports: Art Department – Adrian Amjadi brought with him to the Committee meeting a large piece of metal from the ceiling in the Marsee Auditorium. Each section holds others in place and when a section is missing there is nothing to hold the other sections in place which could cause them to break away and fall to the floor. Nancy Adler and her staff are very concerned since there is potential for a section to fall 20 feet in the air and land someone. Apparently this issue had both been reported to the Committee and a work order had been submitted to Facilities in the past but nothing was done to correct the problem. Valerie Wagner commented that she did not have knowledge of this safety issue until today. Robert Brobst requested support documentation as to prior work orders submitted concerning this safety concern. Adrian Amjadi reported of a potential safety hazard for a small child to fall through the slants on the staircase leading to the balcony on the 2nd level of the Auditorium. Valerie Wagner recommended for Facilities to take a look at the area in order to come up with a solution. Adrian Amjadi also reported that someone broke our door at the Ceramics loading dock area. I came in one morning and it looked like someone just barreled through the door. The top of the door has been broken off and will no longer close. I was informed by Campus Police, that they had submitted a work order to have door repaired. Robert Brobst stated that Facilities did not have a work order to have door replaced. Valerie Wagner instructed Adrian Amjadi to go ahead and submit a work order to have this corrected. SAFETY & HEALTH COMMITTEE Minutes of Wednesday, May 28, 2014 Page 3 of 3 Facilities - Robert Brobst reported since the STEM Center is now open, the Office of Safety and Health should assist the Center in making sure that exit signs and other require signage is installed. I had also asked Arturo Hernandez to request the signage. There is also a problem with the huge window in the Director of STEM Center’s office. The Director has made a compliant that there is too much natural light coming into his office and he has requested some type of cover for the window for privacy issues. 7.
