Department CPR Finding: Documentation review and staff interviews indicated that physical
education is optional for students in grades 11 and 12.
Narrative Description of Corrective Action: The High School Principal, in consultation with the
Superintendent, is currently developing a plan that will meet state requirements regarding
physical education as a graduation requirement for student in grades 11 and 12.

By May 18, 2011, the district will submit documentation evidencing that the district’s proposed
plan of corrective action will be implemented at the start of the 2011-2012 school year. Submit a
revised draft of the 2011-2012 Hatfield Smith Academy High School course of study that includes
mandatory physical education in grades 11 and 12.

Criterion & Topic: CR 10A Student handbooks and
codes of conduct
Department CPR Finding: Documentation review indicated that student handbooks and codes of
conduct for both Hatfield Elementary School and Smith Academy do not contain appropriate
procedures for the discipline of students with special needs and Section 504 Accommodation Plans.
Additionally, student handbooks and codes of conduct do not reference M.G.L. c. 76, s. 5. Student
handbooks do not contain the school’s procedure for accepting, investigating, and resolving
complaints alleging discrimination or harassment and do not contain the disciplinary measures that
the school may impose if it determines discrimination or harassment has occurred.
Narrative Description of Corrective Action: Student handbooks at both the elementary school
and the high school have now include appropriate procedures for the discipline of students with
special needs and Section 504 Accommodation Plans. In addition, both handbooks now contain
the school’s procedure for accepting, investigating, and resolving complaints alleging
discrimination or harassment and do not contain the disciplinary measures that the school may
impose if it determines discrimination or harassment has occurred. The handbooks have been
reviewed and accepted by the School Committee.

Department Order of Corrective Action: On September 20, 2010, the district submitted revised
2010-2011 student handbooks for the elementary and high school levels. The Department has
reviewed these. All handbooks include appropriate procedures for the discipline of students with
special needs and Section 504 Accommodation Plans. The high school handbook includes
procedure for accepting, investigating, and resolving complaints alleging discrimination and
harassment.

procedure for accepting, investigating, and resolving complaints alleging discrimination for the
elementary school student handbook that have been approved by the School Committee. Submit
the school committee minutes documenting this approval. The district will also report how
this information is currently provided to parents/guardians, students, and staff.
