Application Checklist
Section I- Instructions:

Thoroughly review, understand and complete each item on Pages 2 & 3 of this
document.

Check off each line item on Page 2 as you complete the requirement.

Submit Pages 2 & 3 of checklist along with the other documents you are required
to turn in.

Retain a copy of all submitted documents for your personal records.

Due to privacy considerations, all documents pertaining to this application should
be submitted via fax or USPS to protect the applicant’s personal information.

E-mail is not a valid form of document submission.

You will be notified via phone and e-mail within 2 business days of your
application’s arrival at SSU NROTC.

You may contact LT McClinton with any questions or to check receipt of your
application.

Section II- Checklist:
 1. Review Scholarship Fact Sheet. Ensure you meet all requirements for the
MSISR.
o If you do not meet all requirements, explain your plan for correction of any
discrepancies in the “Remarks” section of this checklist.
Sample Remarks: “I have a 400 on the Mathematics portion of the SAT. I
intend to retake the SAT in January 2014.”
 2. Complete and submit the MSISR Application. **
 3. Complete and submit the Statement of Understanding (last page of MSISR
Application). ***
 4. Complete and submit the Debarment and Suspension Statement. ***
 5. Complete and submit the Drug Statement. ***
 6. Submit a copy of your high school transcript. This does not need to be an
official copy.
 7. Submit a copy of your college transcript (if any college credit earned). This
does not need to be an official copy.
 8. Submit a copy of your SAT/ACT scores. Either make a copy of the official
scores mailed to you or print your scores from the official test website.
 9. Submit one letter of recommendation from a teacher, supervisor, counselor,
mentor, etc. on your behalf.
 10. Submit a copy of your birth certificate. This does not need to be a copy
with a raised seal.
 11. Submit a photo of yourself in khaki pants and a plain, short-sleeved, polostyle shirt (See example attached to the e-mail you received.)
 12. Submit a photo of each tattoo on your body and the significance/ meaning
of each. (See example attached to the e-mail you received.)
** Special Instructions: Disregard directions on Page 26 to submit application to
Mr. Melvin Curry or any other recruiter. Submit application to LT McClinton in
accordance with the instructions in Section I of this document.
*** Special Instructions: A guidance counselor, JROTC instructor or
parent/guardian can serve as a witness as required for these forms.
