Close calls, near misses, and what they tell us about managing risk ____________________________________ Social work is a profession which manages risk and, occasionally, we get it wrong. Inquiry reports propose new solutions which frequently involve changes to the regulatory framework and aspiration to generate rule compliance. Research from a number of regulated industries seeking ‘early warning’ systems for the management of risk lead us to consider the epistemology of failure.
