Dear,
This letter is to inform you that you are on Level 4: Pre-dismissal Academic Probation. This means you
have had a cumulative GPA below 2.0 for four consecutive quarters. This is the last quarter you will be
allowed to register at De Anza College if you don’t earn at least a cumulative GPA of 2.0 after your
spring quarter classes. As a result of your probation status, your priority registration has also been
negatively affected. Your success this quarter may help you regain your priority registration status.
Please reply to this email address DAL4@deanza.edu with your student ID letting us know how you plan
to ensure you will pass your classes this quarter. You may also ask any questions regarding your
academic probation. A counselor will respond to your email within a few days.
Please review the Student Success Workshop PowerPoint that was presented in the Student Success
Workshop for information on probation and strategies to improve your GPA.
You are encouraged to come to the Counseling Center to make an appointment to meet with a counselor
to update your student education plan in Degree Works. The Counseling Center is committed to your
academic success.

quarters (excluding summer) will be dismissed from De Anza for one quarter.
