Dear, This letter is to inform you that you are on Level 3: Severe Academic Probation. This means you have had a cumulative GPA below 2.0 for three consecutive quarters. There is a HOLD on your registration for fall quarter 2016. This HOLD will also prevent you from using any services related to registering, adding, dropping class(es) or obtaining grades, transcripts, certificates and degrees. As a result of your probation status, your priority registration has also been negatively affected. Your success this quarter may help you regain your priority registration status. In order to have your HOLD removed you must: 1. Print the Academic Progress Report form and have each of your instructors record the grade you are currently earning. If you are taking online courses please have your instructor email you a written evaluation including your current grade and then print the email. 2. Make an appointment to meet with a counselor to develop a plan of action for fall quarter. You must bring your completed Academic Progress Report. Counseling appointments fill quickly so make your appointment as soon as possible.
