YES Initial Conference Date ________ Review Date __________ Students participating in a Magnet program who are experiencing difficulties in meeting the Magnet Program Entrance Agreement for STEM-Related Magnet Theme program standards are placed on a Magnet Program Growth Plan for a minimum of one grading cycle. A student should be placed on a growth plan after initial steps have been taken to assist the student at the first indication of struggle. The growth plan is intended to help students meet program expectations as outlined in the Magnet Program Entrance Agreement for STEM-Related Magnet Theme. A growth plan committee comprised of the student’s teachers, the Magnet Coordinator and an administrator will evaluate the student’s progress on this plan at the end of the specified time period. Courses Addressed: PLAN OF ACTION Identified problem: Sub-standard grades Incomplete/Missing Assignments The student will: Record all of the following items in his/her agenda book: -All assignments for each class -All grades received in each class -Attendance at tutorials at least once per week for every class below 80. (Have the teacher sign that day in your agenda book) Be present and attentive in all classes from bell to bell. Revised 1/7/14 ___ Behavior (Describe): When the student is called to the magnet office for a progress check, he or she must bring the agenda book with all necessary documentation. Page 1 of 2 The parent/guardian will: -Maintain communication with teachers -Monitor grades and attendance via Gradespeed -Review graded and returned work -Initial next to completed homework in Agenda notebook each evening. -Submit excuse notes to the attendance office within three (3) days of a student’s absence. The teacher(s) will: -Provide tutorials outside of class time, upon request of student. -Put a minimum of two new grades into Gradespeed every week. PLEASE NOTE: The Magnet transfer is a one year commitment; however, the recommendation from the committee could result in the student losing their place in the Magnet program at the end of the year and being placed in the regular program at the student’s zoned school at the beginning of the next school year. Students placed in an alternative school (DAEP) will enroll in their zoned school upon completion of their alternative placement. Students cannot be placed in the regular educational program on the same campus where they have a Magnet transfer. No other transfers of any kind may be granted to another school. Should the child choose to leave the program voluntarily before the end of the school year, he or she may return only to their zoned campus. A voluntary exit form must be completed if a student withdraws from the program before the end of the year.
