Staff Directions for Team Implementation Checklist (TIC)- ONE survey per
team.
Go to www.pbssurveys.org
Click on "surveys" in the top row of the site (above the part that says
Welcome to PBS Surveys)
Scroll down and select Team Checklist- a menu will appear on the right
Select complete checklist
Select Respondent in Type of Login
Enter School Account Number – 6 digit # used by everyone in the
building
Click- Login
Select your occupation/role and name then proceed with survey by
clicking on the bubble that most accurately reflects your thoughts.
Make sure to complete both the start-up activities and on-going activities
questions. Directly above the first question is the tab to select on-going
activities after you have completed the first page of questions.
