NOTICE: Completion of this form on your computer does not register your bike. You must follow the instructions
below to receive a registration sticker.
Instruction:
1. Complete the form on your computer; be sure to provide all the information requested. Every bike must have a
serial number or an owner applied number. Forms submitted without one of these numbers will not be
accepted. Public Safety staff can help you locate these numbers if you need assistance.
2. Print the form.
3. Bring the completed form and your bike to the Department of Public Safety at Cascade Hall, room 200. We will
provide you with a bike registration sticker for free, which you can put on your bike frame.
