Agenda for Change Agenda for Change (AfC) was implemented in the NHS, across the UK in December 2004. It was the biggest overhaul of NHS-wide pay, terms and conditions in over 50 years. It applies to all NHS organisations and therefore sets a UK framework for pay, terms and conditions of employment. The role of the fertility nurse is to provide an holistic approach to fertility investigation, treatment, and early pregnancy through compassionate, informed and evidence-based practice. Fertility nurses work as part of multi-professional teams whose focus is on delivering a high standard of care. Research plays a fundamental part in this process. AfC and its Knowledge and Skills Framework (KSF) means that all staff will have clear and consistent development objectives; can develop in such a way that they can apply the knowledge and skills appropriate to their level of responsibility; and are helped to identify and develop knowledge and skills that will support their career progression. The changing context This field of nursing is highly specialised, and subject to dramatic change as innovations and new techniques are adopted - many, previously the responsibility of medically qualified staff. The RCN Midwifery and Fertility Nursing Group (FNG) believes that professional advice and support are required for nurses developing their roles in a dynamic and rapidly advancing field, which brings with it challenging moral dilemmas (Barber, 1997). Under AfC jobs are evaluated using a bespoke NHS job evaluation scheme. This gives each job a 'weighting' that then determines where each job slots into the new pay bands. Common 'job profiles' continue to be developed and are applicable across the UK, and where a job fits a profile it is possible to place it straight onto an appropriate new pay band. For the relatively few jobs that don't automatically fit a profile, trained job evaluators drawn from management and staff side carry out the evaluation using a Job Analysis Questionnaire (JAQ). Each pay band has a number of pay points. Staff below the maximum point can expect to progress to the next point each year. The competence framework has been developed in this context, as well as taking into account other professional and political factors such as: ✦ Agenda for Change (DH, 1999) including the Knowledge and Skills Framework and the NHS Job Evaluation Scheme ✦ need for leadership in specialist nursing ✦ need for the development of standards ✦ NHS Plan (DH, 2000) and its equivalent in Scotland, Wales and Northern Ireland ✦ increased focus on work-based and lifelong learning plus supervision ✦ changing focus towards professional rather than academic accreditation ✦ national service frameworks ✦ increasing patient and user expectations ✦ Future Nurse (RCN, 2003). There are two points on each pay band called gateways where staff knowledge and skills are assessed using the Knowledge and Skills Framework (KSF). Pay progression at the gateways is linked to the demonstration of applied knowledge and skills to support continuing professional development. The presumption in the KSF is that staff will pass through these gateways unless there are reasons as to why they shouldn't. By focussing on the specific care treatments used in fertility nursing, the document gives nurses across the UK the support they need to achieve the skills and knowledge necessary to provide a high standard of care.You should use the framework as a developing, empowering and aspirational tool, as well as a means of managing performance as a part of professional development and appraisal schemes. The framework can also be used with other frameworks that highlight core nursing skills and competences. ✦ increased effectiveness to service provision ✦ improved access and choice for care provision. The competences The competences are aligned to the Skills for Health career framework health levels. These are based on classifications used in the KSF (DH, 2004) and the European qualification framework for lifelong learning (SfH, 2006). The levels are for guidance only and are not the same as the Agenda for Change pay bands, which do not consider freedom to act or managerial responsibility. As a practitioner you will be required to produce evidence for each competence in order to demonstrate that you have achieved the competence at the identified/desired level. Forms of evidence that you can use include case histories, self-appraisal via a reflective diary, 360 degree feedback, verification of practice and structured observation of practice. Given the specialist nature of fertility nursing, delineating competences is challenging and depends on the background, composition and ethos of individual teams. In some fertility units the role of fertility nurses is extensive. They have significant involvement in all aspects of fertility work, demonstrating a high level of technical ability and skill. In other organisations, the nurse’s role is limited to assisting other team members. Benefits of the framework Our approach to describing competences has been to accept that individuals’ knowledge and skills improve with time, experience and education, or as their specific role dictates. The competences described in each area need to be considered as a guide rather than a prescription. The role of the unregistered practitioner is limited in this specialist field of care. For the comparatively new consultant nurse the role is not yet well defined across fertility nursing, so the competences described vary from role to role. The competence framework provides benefits for nurses, their employers, patients and the public. Nurses benefit because it helps you to: ✦ deliver consistently high standards of care ✦ identify your level of practice and plan your career in a more structured way ✦ pinpoint personal education and development needs ✦ realise your potential more effectively ✦ seize opportunities to influence the direction of nursing. The competence levels The framework has five levels of competence that can be cross-referenced with the Career Framework for Health as devised by Skills for Health (Skills for Health, 2006). The competences are detailed for each specific intervention, therapy or treatment. Employers benefit because it provides: ✦ a model to ensure consistently high standards of care ✦ clearer insight into the expertise and competence of staff. For example, in assessment of risk management. There are a number of ways that you can use the framework to develop your knowledge and skills, for example, it can be used as a: tool for individual nurses to assess their own competence and training needs ✦ formal appraisal system to assess competence and identify training needs ✦ means of planning an individual nurse’s career pathway ✦ method of developing a comprehensive induction programme for new staff ✦ framework for the development of individual portfolios ✦ follow-up In this way there is a visible coherence and logic to the evolving competences that nurses need to support their patient/clients. Each competence described builds on the one before it because knowledge and skills are acquired cumulatively. Research plays a major part in the role of the fertility nurse and clinical applications change accordingly. This is incorporated into each stage of the client pathway. Using the framework ✦ investigations treatment However, using this approach does mean that some generic competences, for example, those relating to documentation, are omitted. They are at the end of the following text. Fertility nurses working in the UK should be familiar with all local protocols/standard operating procedures (SOP), and national guidelines and regulation. Knowledge and skills framework Nurses should use the specialist competences with the RCN Core competences that are relevant to their role. Practitioners who work in the NHS should use the KSF dimensions and levels of the core framework that relate to the specific KSF outline for their current post. source of information for nurses considering a career in fertility nursing practice. The building blocks for professional development To help you apply the specialist competences to your role, the relevant KSF dimensions and levels have been identified against each area of competence. The KSF abbreviated reference has been used, for example, HWB1 level 4. These are based on the dimensions and levels identified in the RCN Core competences that relate to the level of practitioner, and which refer to competent, experienced and senior practitioners/nurses.Work on the consultant nurse role will be completed shortly, and has yet to start on the health care support worker. The clearly defined competence levels make it possible for fertility nurses to identify their level of practice. The framework gives you the ability to plan your career in a more structured way, and supports your continuing professional development by pinpointing individual development and training needs. By developing competences for fertility and other nursing specialisms, the RCN is providing the key building blocks for improving professional practice in nursing. This will raise the quality of care, and ensure that patients and employers benefit from consistently high standards. These competences can also be used with other dimensions and levels where they are appropriate. For example, where a competence involves undertaking an intervention, the suggested KSF dimension would be HWB7 (interventions and treatments).Although the competence could also be appropriate for KSF dimension HWB5 (provision of care to meet health and wellbeing needs) if providing care is the main focus.We have to be careful because many procedures, such as embryo transfer or pregnancy scan, are undertaken by fertility nurses on their own, and the care/intervention role boundaries are demarcated. If we always assume the caring role, fertility nurses may undersell themselves in the KSF framework. Patient/client pathway In describing the competences, we have used the patient/client pathway as the framework for the work – moving from the causes of infertility, through investigation, into treatment and then follow-up: 4 ROYAL COLLEGE OF NURSING National occupational standards Producing evidence National Occupational Standards (NOS) and National Workforce Competences (NWC) have been developed by Skills for Health following consultation with experts across the health care sector. They describe the scope of the competence, the required knowledge and the standard of performance needed to demonstrate competence. They focus purely on health care functions, not the professional approach such as attitudes and behaviours, or what a person is like.Appropriate NOSs and NWCs have been identified by reference alongside the relevant competences we describe. This means that you can identify an applicable NOS/NWC that may support your development and attainment of competence in the relevant function. The reference code for relevant NOS/NWC is given for each competence. You are responsible for gathering evidence for each competence in order to demonstrate that you have achieved it at the identified/desired level. Forms of evidence that you can use include: ✦ case histories ✦ self-appraisal via a reflective diary ✦ 360-degree feedback ✦ verification of practice and structured observation of practice ✦ a log book ✦ up to date personal portfolio. When you gather evidence it is important that you consider the following: Professional accountability The NMC Code of professional conduct: standards for conduct, performance and ethics (2004) states: ✦ what is the competence statement asking of you? ✦ is the existing evidence appropriate? For example, certificates of attendance at training events are insufficient to demonstrate knowledge or competence in practice.You will need to demonstrate the knowledge that you have gained and how you use this in practice.Assessment of competence in practice also requires supervised practice by a competent practitioner until you both agree that competence has been demonstrated ✦ what else do you need to do to develop evidence? For example, consider reflection and feedback on practice ✦ do you have any further development needs, and how could these be met? ✦ could the evidence that you have be used to cover several competences? For example, one case study may demonstrate that you have used a variety of knowledge and skills in caring for a patient. In this instance you should be able to measure the evidence against several competences. “As a registered nurse, midwife or specialist community public health nurse, you are personally accountable for your practice. In caring for patients and clients, you must: ✦ respect the patient or client as an individual ✦ obtain consent before you give and treatment or care ✦ protect confidential information ✦ co-operate with others in the team ✦ maintain your professional knowledge and competence ✦ be trustworthy ✦ act to identify and minimise risk to patients and clients.” The code goes on to state that as a practitioner: “You are personally accountable for your practice. This means that you are answerable for your actions and omissions, regardless of advice or directions from another professional.” It also goes on to state: “You have a duty of care to your patients and clients, who are entitled to receive safe and competent care”, and that “you must adhere to the laws of the country in which you are practising.” 5 RCN COMPETENCES – FERTILITY What is evidence? It is recognised that these competences will evolve over time – as they are used in practice and also as practice changes to reflect new thinking. The role of the consultant nurse, in particular, will evolve and expand.
