Arrests In Tampa PIP Fraud Scheme
The Florida Division of Insurance Fraud (DIF) recently announced the arrests of
seven Tampa residents following a two-year PIP (Personal Injury Protection)
investigation into fraud involving Integrated Healing Clinic in Tampa. The
investigation discovered that clinic staff had filed several patient claims with
multiple insurance carriers for prices that far exceeded actual treatment costs
and, in some cases, for treatments that never took place.
“These individuals orchestrated a complex operation to defraud insurance
companies and, ultimately, all Floridians,” said Deputy CFO Jay Etheridge. “I’m
proud of the results of this collaborative investigation and hope this serves as a
deterrent for anyone considering or currently engaged in similar criminal acts in
our state.”
This extensive investigation revealed that clinic employee Minette St. Fleur, 50,
operated as a recruiter, paying patients to go along with the scheme to file
fraudulent claims. She was arrested and charged with insurance fraud and
patient brokering.
The following patients were also arrested and charged with false and fraudulent
insurance claims; Sonthonax Ferdinand, 57; Marie Jean Gilles Valcin, 42; Marie
Celestin, 38; Josue Aguste, 42; Elisena Louissaint, 48; and Joseph D Pierre, 39.
Integrated Healing Clinic was incorporated in 2010, listing Samar Fadda as the
sole official. Fadda has been licensed as an acupuncturist in Florida since 2008.
The National Insurance Crime Bureau and several insurance companies were
instrumental to the success of the investigation, including Geico, 21st Century
Insurance, Direct General Insurance, Progressive, Safeco Insurance, State Farm
Insurance and Liberty Mutual Insurance. The case will be prosecuted by the
office of Hillsborough County State Attorney Mark A. Ober.
Anyone with information about suspected insurance fraud is asked to call 1-800378-0445. Citizens who provide tips can remain anonymous. The Department of
Financial Services has awarded $365,000 to 63 citizens as part of its Anti-Fraud
Reward Program. The program rewards individuals up to $25,000 for information
that directly leads to an arrest and conviction in an insurance fraud scheme.

Rafael Delgado Promoted to DIF Major
The Division of Insurance Fraud (DIF) recently announced the promotion of Rafael Delgado from Captain to
District II Major, effective April 29, 2015. Major Delgado will supervise the South and South Central Regions,
replacing retiring Major Glen Hughes.
Major Delgado is a career law enforcement veteran with more than 30 years of experience. He served 21
years with the New York City Police Department on uniformed patrol, organized crime control bureau,
detective bureau and assignment on an FBI task force unit, before retiring in 2002.
In 2005, Delgado moved to South Florida and renewed his law enforcement career as a detective with DIF,
assigned to the Miami Field Office. He started on a general fraud squad and later moved to a personal injury
protection (PIP) squad. In August 2011, he was promoted to lead detective of the PIP squad. He is a
certified field training officer, and for several years, he had an active role in training new law enforcement
detectives assigned to the Miami Field Office.
In March 2012, Delgado was promoted to lieutenant of the Miami workers’ compensation squad. In
September 2012, Delgado transferred to take command of one of the three PIP squads in Miami. In April
2014, Delgado was promoted to Captain of the South Central Region, supervising five squads located in Ft.
Lauderdale, Ft. Myers and West Palm Beach.

Details:
The Florida Department of Financial Services, Division of Insurance
Fraud (DIF) is attempting to locate Katherine Super in reference to the
outstanding warrant.
While working as an office manager at a chiropractic clinic, Super
schemed to file false and fraudulent billings on behalf of clients for
medical treatments that did not occur, to monetarily gain from personal
injury protection benefits.
Super has yet to be found since her warrant was issued in May 2014.
Her warrant has nationwide extradition.

Court Denies Attorney’s Challenge to Constitutionality of Solicitation Statute
Attorney Brian Greenspoon, arrested by the Division of Insurance Fraud in October 2014 as part of Operation Cold Call,
challenged the constitutionality of the Florida Statute regarding solicitation of accident victims.
Greenspoon filed a motion in Palm Beach County Circuit Court to dismiss his case based on that argument, which was
denied by Judge Karen Miller.
The challenge was defended by Assistant State Attorney Ryan Kelly.
James Feeney Promoted to Lieutenant of WPB Auto Fraud Squad
The Division of Insurance Fraud (DIF) announced that James Feeney was promoted from lead detective to Lieutenant of
the West Palm Beach Auto Fraud Squad in the South Central Region, effective April 29, 2015.
Lt. Feeney joined DIF more than four years ago, bringing with him an extensive background in law enforcement
investigations and training. He served on the Jupiter Police Department as a patrol commander, sergeant, detective and
officer, while fostering deep community connections on the neighborhood enhancement team. Prior to that, he gained
experience with the Palm Beach County Sheriff’s Department. Lt. Feeney is an instructor at the Palm Beach Community
College on topics covering organized crime, weapons of mass destruction awareness and police protective measures.
Lt. Feeney received a Bachelor of Arts in Criminal Justice in 1993 from Florida Atlantic University and a Bachelor of
Public Management degree in 2005 from Florida Atlantic University.

Leader of Colorado Insurance Fraud Ring Sentenced to Prison
The leader of a Colorado insurance fraud ring was sentenced to prison in April. Mike Tee, 41, will spend 18 years in the
Department of Corrections for masterminding a criminal ring that stole insurance money by staging automobile
accidents. Tee and his associates would claim both vehicle damages and bodily injury in fake accidents to collect
insurance payouts. As part of the thefts, Tee and the others claimed the vehicles they owned were stolen, when in fact
they were not and insurance companies ultimately paid more than $80,000.

Allstate Donates Vehicle to Cape Coral Police Department
In April, Allstate Insurance Company donated a vehicle to the Auto Theft Unit of the Cape Coral Police Department to
assist in their efforts to fight insurance fraud. This donation was made possible with the assistance of National Insurance
Crime Bureau (NICB) Special Agent Frank Napolitano.
